lntrodution: Biventricular pacing is a new therapy for patients with congestive heart failure. We present the results of the implantation and follow of the first four patients with cardiac transvenous resynchronization in Costa Rica. Methods: All the patients was men, the age was 66 years in average. Two of them was in funcional class IV and the others in class III. The principal etiology was isquemic heart disease. The end diastolic VI diameter in average was 63 mm. The follow-up in average was 6 months. Results: From the 3 months after implantation all the patients improve their funcional class and their ejection fradion (25% to 33%). The QRS width was 135 msec before and 95 msec after the resynchronization. There no were complications. Clonclusion: The ventricular resynchronization is a new therapy that we have now in our country, for some patients with cardiac heart failure.
