The sinus of Valsalva aneurysms are dilations of the aortic wall located between the aortic valve annulus and the sinotubular junction of the ascending Aorta. Their designation comes from the corresponding coronary sinus (right, left or non-coronary). It is a rare clinical entity that may be congenital (most frequent) or acquired. It is most common complication is rupture into the right heart chambers, after which, unfortunately, the majority are diagnosed. There are however, those diagnosed before rupture as an incidental finding detected in imaging tests for other purposes or during cardiac surgery of another lesion. Unlike the ruptured aneurysm, which has a median survival of 3.9 years, the natural history of an un-ruptured aneurysm is unknown. To date, a consensus about the best surgical repair technique has not been reached, however decision-making is possible guided by experience-based practices in more experienced centers. We present a literature review inspired by a patient diagnosed with an un-ruptured sinus of Valsalva aneurysm at a hospital in San José, Costa Rica.
