Hepatitis B and Hepatic Transplant

Resumen: El trasplante hepático es una opción de tratamiento para los pacientes con enfermedad hepatica terminal por HBV. El uso de HBIg disminuye el riesgo de reinfección.

Abstract: The liver transplantation is one of the possible treatments for those end-stage patients with liver diseasse for HBV. The use of HBIg lowers the risk of reinfection.

Indicaciones de trasplante hepático en pacientes con

Hepatitis B (HBV)

La expectativa de vida es < a 2 años; historia de peritonitis bacteriana espontánea; encefalopatía crónica; ascitis refractaria; sangrados digestivos a pesar de tratamiento endoscópico de várices.

Recurrencia de HBV en pacientes trasplantados

Estos porcentajes de recurrencia se presentan en pacientes sin tratamiento para HBV que fueron 67% pacientes trasplantados por cirrosis; 40% pacientes trasplantados por hepatitis fulminante B-Delta y el 32% de los pacientes trasplantados por HBV fulminante.

El porcentaje de recurrencia está relacionado con la replicación viral por lo que utilizando las siguientes mediciones: HBeAg y HBV-DNA en suero al momento del trasplante dando los siguientes resultados: 83% en HBV DNA y HBeAg positivo en pacientes cirróticos y el 58% en HBV DNA y HBeAg negativo en pacientes cirróticos.

Prevención de la recurrencia de Hepatitis B postrasplante.

Hubo diferentes protocolos, los cuales mostraron recurrencia, hasta que el grupo de Hannover estableció dosis de HBIg para mantener los títulos de anti HBs mayores 100 IU/L por un periodo no menor de 6 meses. El grupo de París y otros centros de Europa adoptaron la inmunoprofilaxis indefinida.

Tratamiento

El tratamiento para pacientes es de: 10000 UI HBIg IV fase anhepática; 10000 UI HBIg IV durante 6 días postoperatorios; nivel de anti HBs se toma semanalmente cuando es <100 IU/L se administra una dosis de HBlg. Figura 1

La evidencia reciente ha demostrado que el uso de inmunoglobulinas a bajas dosis puede ser eficaz si se sigue inmediatamente posterior al trasplante 800Ul diario por 8 días intramuscular, luego 800Ul cada mes según medición de niveles de anticuerpos.
