We present a case with a patient who attends at emergency department with cellulitis, that develops fulminant septic shock and dies before 24 hours of the arrive. The microorganism was identified like Vibrio cholerae non-01-non-O139. The patient only was identified with liver disease. It is the first case described in Costa Rica, although the bacteria is rare, it is necessary to consider it in differential diagnoses considering its high virulence.
