La PRL se realizó con colocación de 4 trócares luego de poner la cámara en cicatriz umbilical. El tiempo operatorio total fue de 240 y 205 minutos, la pérdida de sangre estimada fue de 150 y 100 ml respectivamente. Se extrajeron las próstatas con vesículas seminales por una incisión umbilical de 3 cm. Un dreno cerrado con succión se externalizó por una de las incisiones de los trócares de 5mm. No hubo complicaciones trans ni post operatorias. La estadia hospitalaria post operatoria fue de 3 y 2 días, el dreno se retiró al tercer y segundo día post operado y las sondas Foley se retiraron a los 14 días. Las próstatas pesaron 27 y 23 gramos, los puntajes Gleason fueron de (3+4) y (3+3) ambos tuvieron márgenes negativos, sin infiltración vascular ni nerviosa. La PRL es un procedimiento mínimamente invasivo para hombres con cáncer de próstata órgano confinado realizable, seguro y reproducible. Con mejor resultado cosmético y menor dolor posoperatorio.]]></p></abstract>

Two 50 and 64-year-old patients respectively, with an adenocarcinoma of the prostate with Gleason scores (3+ 3) 6, diagnosed trough transrectal ultrasonografic guided biopsies, indicated by an elevation of the prostate specific antigen (PSA) or abnormal rectal digital examination (DRE). The LRP was performed transperitonealy with Montsouris technique using standard laparoscopy instruments and high definition laparoscopic video tower. After placing a 10mm camera port through the umbilicus, the procedure was accomplished with 4 trocars. The entire operative time was 240 and 205 minutes, whereas the blood loss was 150 and 100 ml respectively. The extraction of the specimens with seminal vesicles was through a 3 cm. umbilical incision. A closed drainage system was left in place through one of the 5mm port incisions. There were no complications trans or post operative, the time of hospitalization was 3 and 2 days respectively, and the drain was retrieved third and second day postoperative. Moreover, an indwelling Foley catheter was removed 14 days in an outpatient setting. The pathology department reported that both prostates weighed 27 and 23 grams each, and had a Gleason score (3+ 4) 7 and (3+3) 6 respectively. In addition, both specimens were reported to have negative margins, without neurovascular infiltration. In conclusion, LPR is a safe, feasible and reproducible a minimally invasive procedure for men with confined prostate cancer, with better cosmesis, less bleeding and less postoperative pain.]]></p></abstract>

La cirug&iacute;a radical de pr&oacute;stata sigue siendo la primera opci&oacute;n para el tratamiento del c&aacute;ncer en etapas tempranas, haci&eacute;ndose la primera prostatectom&iacute;a radical a nivel mundial 1904. Desde entonces, la t&eacute;cnica quir&uacute;rgica ha evolucionado y se ha incorporado la cirug&iacute;a m&iacute;nimamente invasiva.</font></p>     <p><font face="Verdana" size="2">La primera prostatectom&iacute;a radical laparosc&oacute;pica (PRL) se realiz&oacute; en Europa en 1997, luego pas&oacute; a Estados Unidos de Norteam&eacute;rica y actualmente se realiza en muchos pa&iacute;ses del mundo<sup>14</sup>.</font></p>     <p><font face="Verdana" size="2">Las principales ventajas de este procedimiento de invasi&oacute;n m&iacute;nima son: menor dolor postoperatorio, recuperaci&oacute;n m&aacute;s r&aacute;pida, estancia hospitalaria m&aacute;s corta, heridas m&aacute;s cosm&eacute;ticas con adecuados resultados oncol&oacute;gicos, y con continencia y funci&oacute;n sexual comparables a los de la cirug&iacute;a abierta.</font></p>     <p><font face="Verdana" size="2">Se describen a continuaci&oacute;n los primeros casos de PRL transperitoneal2 con la t&eacute;cnica Montsouris<sup>6,7,12,13</sup> para c&aacute;ncer de pr&oacute;stata, realizados por m&eacute;dicos costarricenses en el Hospital Dr. Rafael A. Calder&oacute;n Guardia</font></p> <b><font face="Verdana" size="3">     <p>Caso Cl&iacute;nico</p> </font></b><font face="Verdana" size="2"> </font>     <p><font face="Verdana" size="2">La PRL se realiz&oacute; a 2 pacientes de 50 y 64 a&ntilde;os de edad, ambos con adenocarcinoma de tipo acinar com&uacute;n con puntajes Gleason pre quir&uacute;rgicos a 3+3, con un estadio cl&iacute;nico T2a y ant&iacute;geno prost&aacute;tico inicial de 8,66 y 1,29 ng./ml, respectivamente con adecuada continencia y SHIM &gt;21.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Para el procedimiento se utiliz&oacute; torre de video laparosc&oacute;pica de alta definici&oacute;n, insuflador de CO<sub>2</sub>, generador ligasure, unidad electro quir&uacute;rgica (monopolar/bipolar), tr&oacute;cares descartables tres de 5mm y dos de 10 mm , pinzas de agarre de 5 mm, c&aacute;nula de succi&oacute;n-irrigaci&oacute;n de 5 mm., dos porta-agujas laparosc&oacute;picas rectas, tijeras Metzenbaum reutilizables, electrodo monopolar de alta frecuencia tipo gancho en L de 5mm., pinza de corte y coagulaci&oacute;n ligasure de 5 mm., aplicador de clips tipo Hem o lock de 5 y 10 mm, clips de Hem o lock, buj&iacute;a uretral met&aacute;lica (Beniqu&eacute;) 24 French, bolsa de extracci&oacute;n laparosc&oacute;pica, sonda Foley 20Fr, gu&iacute;a met&aacute;lica de sonda uretral, suturas: Vicryl 2 ceros con aguja CT-1 (ligadura del complejo venoso dorsal); Biosyn 2 ceros con aguja GU-46 (anastomosis uretrovesical), disector en &aacute;ngulo recto de 10 mm y dreno Jackson Prat con colector.    <br> </font></p>     <p><font face="Verdana" size="2">    <br>     <br> </font></p>     <div style="text-align: center;"><img src="/img/revistas/amc/v52n4/a13t1.gif" title=""  alt="" style="width: 332px; height: 307px;">    <br>     <br> </div>     <p><font face="Verdana" size="2">Durante la cirug&iacute;a se administr&oacute; a los pacientes anestesia general y se colocaron en dec&uacute;bito dorsal con Trendelenburg. La PRL se realiz&oacute; mediante colocaci&oacute;n de 5 tr&oacute;cares: dos de 10 mm y tres de 5 mm, en el ombligo y en ambas fosas iliacas. Se abrieron dos ventanas separadas peritoneales con gancho monopolar por encima de la vejiga, abordando el espacio de Retzius. Se retir&oacute; grasa de ambas fascias endop&eacute;lvicas, se abordaron ambos "Velos de Afrodita" para preservar bandeletas (interfascial), se retir&oacute; la grasa sobre la pr&oacute;stata, se seccionaron los ligamentos pubo-prost&aacute;ticos bilaterales, se coloc&oacute; un punto proximal y se lig&oacute; el complejo venoso dorsal con Vicryl 2 ceros. Se seccion&oacute; el complejo dorsal y la uretra con tijeras, se inici&oacute; la disecci&oacute;n tal como se describe en la t&eacute;cnica de Montsouris6,7,12,13, la cual combina disecci&oacute;n retr&oacute;grada inicial y anter&oacute;grada posterior. Se separ&oacute; la uni&oacute;n pr&oacute;stata-vejiga, ves&iacute;culas seminales y los conductos deferentes (ver <a href="#fig1">Figura 1</a>).    <br> </font></p>     ]]></body>

Se coloc&oacute; un dreno, se retiraron los tr&oacute;cares con visi&oacute;n laparosc&oacute;pica y se cerraron las herida de las incisiones realizadas.</font></p>     <p><font face="Verdana" size="2">Las cirug&iacute;as se realizaron totalmente por v&iacute;a laparosc&oacute;pica. El tiempo operatorio fue de 240 y 205 minutos respectivamente, y el sangrado aproximado fue de 150 y 100 ml. El tiempo de hospitalizaci&oacute;n postoperatorio fue de 3 y 2 d&iacute;as. No hubo complicaciones intraoperatorias ni postoperatorias. 7 La incisi&oacute;n de la piel en la cicatriz umbilical fue de 3 cm, los paciente recibieron antiinflamatorios no esteroides en las primeras 36 horas postoperatorias. Los drenos Jackson Prat se retiraron a los tres y dos d&iacute;as postoperatorios y las sondas Foley 14 d&iacute;as despu&eacute;s<sup>10</sup> (<a href="#cuadro2">Cuadro 2</a>). Las pr&oacute;statas con ves&iacute;culas seminales pesaron 27 y 23 gramos, Gleason 3+4 y 3+3, sin infiltraci&oacute;n de la c&aacute;psula prost&aacute;tica, ni de vasos sangu&iacute;neos, ni de nervios. Los m&aacute;rgenes fueron negativos.    <br> </font></p>     <p><font face="Verdana" size="2">    <br> </font></p>     ]]></body>
<body><![CDATA[<p style="text-align: center;"><font face="Verdana" size="2"><a  name="cuadro2"></a><img src="/img/revistas/amc/v52n4/a13t2.gif" title="" alt=""  style="width: 327px; height: 324px;">    <br>     <br> </font></p>     <p><font face="Verdana" size="2">Los beneficios de la cirug&iacute;a laparosc&oacute;pica incluyen: baja morbilidad, disminuci&oacute;n del sangrado, disminuci&oacute;n del dolor, corta estancia hospitalaria y regreso temprano a las actividades normales.<sup>15</sup> Algunas de sus aplicaciones en cirug&iacute;a urol&oacute;gica son tumores de ri&ntilde;&oacute;n, ri&ntilde;ones no funcionales sintom&aacute;ticos, nefrectom&iacute;a de donante renal, estenosis de la uni&oacute;n pieloureteral, tumores de vejiga con invasi&oacute;n a la muscular y tumores de pr&oacute;stata. Comparado con la prostatectom&iacute;a radical tradicional abierta la cual requiere una incisi&oacute;n de 15 a 20 cm. m&aacute;s otra adicional para la colocaci&oacute;n de un dreno, lo que aumenta el dolor por la herida y dolor muscular de los m&uacute;sculos rectos abdominales al colocar el separador; en la PRL, el dolor y el sangrado son menores, se utilizan cinco puertos y al final se realiza una incisi&oacute;n de 3 cm en la cicatriz umbilical para la extracci&oacute;n de la pr&oacute;stata. Su principal limitaci&oacute;n es que es un procedimiento muy complejo con una curva de aprendizaje larga, la cual requiere de una importante experiencia previa en laparoscop&iacute;a, tanto del cirujano como del primer ayudante, el cual le debe dar los &aacute;ngulos adecuados con la c&aacute;mara. Se espera que conforme aumente la experiencia nacional, los tiempos operatorios y la estancia hospitalaria disminuyan marcadamente.</font></p>     <p><font face="Verdana" size="2">La PRL es un procedimiento m&iacute;nimamente invasivo para hombres con c&aacute;ncer de pr&oacute;stata organoconfinado, realizable, seguro y reproducible.
