Se mantuvo en excelente estado general y afebril, sin poder aislar una bacteria causal, por haberse iniciado antibioticoterapia temprana; fue tratado además con cirugía conservadora, con debridamiento y lavado de tejidos afectados con agua oxigenada y solución yodada, y con derivación de la orina con una cistostomía suprapúbica, para una excelente evolución y resultado final.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[We herein report an uncommon case of a diabetic sailor with renal failure and emphysematous cystitis, who presented with hematuria, dysuria, suprapubic pain and obstructive urinary symptoms, initially treated at sea with antibiotics, with an ultrasound showing the presence of gas. This man remained in good general condition with no fever, but it was not possible to isolate bacteria in the cultures because of early use of antibiotics. He was also treated conservatively with surgical debridement of superficial tissues and bladder mucosa, washings with peroxide and iodine solutions and with a suprapubic tube for urinary diversion. The outcome was excellent with bladder preservation and full recovery of his health.]]></p></abstract>

<body><![CDATA[<br> Las pruebas de VIH y de hepatitis B y C fueron negativas (Cuadro 1), con resultado de ex&aacute;menes de sangre. Un ultrasonido report&oacute; la presencia de gas dentro de la mucosa vesical y se hizo el diagn&oacute;stico radiol&oacute;gico de una cistitis enfisematosa. </font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font> </div>     <div class="Sect"> <font size="+3"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="2">    <br> Una colonoscop&iacute;a fue negativa por diverticulitis y por c&aacute;ncer de colon y no se encontraron f&iacute;stulas vesicoent&eacute;ricas. Un ultrasonido de control, 3 d&iacute;as despu&eacute;s de su ingreso, mostr&oacute; un flem&oacute;n en vejiga y pelvis con severo edema de todas las capas de la vejiga y de la grasa perivesical, con presencia de gas intraparietal. </font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font>     <br> <font size="+3"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="2">    <br> El paciente fue llevado a la sala de operaciones para ser explorado y se palp&oacute; bajo anestesia una gran masa inflamatoria en todo el hipogastrio, que llegaba hasta el ombligo, sin la presencia de globo por retenci&oacute;n urinaria. Se encontr&oacute; un edema y signos de inflamaci&oacute;n muy severos en todo el espesor de la vejiga y de la grasa perivesical, sin presencia de material purulento ni de f&iacute;stulas enterovesicales; se hizo una debridaci&oacute;n del tejido inflamatorio, principalmente de la mucosa vesical, se tomaron biopsias de la vejiga, se coloc&oacute; un tubo de cistostom&iacute;a suprap&uacute;bica, se lavaron los tejidos con agua oxigenada, yodo y una soluci&oacute;n de antibi&oacute;ticos basada en gentamicina y cefalosporinas, y se dejaron drenajes de <i>penrose </i>en los espacios de Retzius y perivesical. </font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font>     <br> <font size="+3"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="2">    <br> En el postoperatorio tuvo edema y celulitis de piel en herida y &aacute;reas de equimosis en el abdomen. El paciente siempre se mantuvo afebril y en buen estado general. Los cultivos de orina y los cultivos por aerobios y anaerobios obtenidos en la sala de operaciones, del tejido inflamatorio, fueron reportados como negativos y nunca se aisl&oacute; una bacteria causal. Las biopsias de vejiga reportaron una cistitis aguda severa enfisematosa. No hubo infecci&oacute;n de la herida quir&uacute;rgica y una semana despu&eacute;s hab&iacute;a mejor&iacute;a con disminuci&oacute;n de la masa en hipogastrio, y desaparici&oacute;n del edema de genitales y miembros inferiores. Sus manifestaciones cl&iacute;nicas var&iacute;an desde los pacientes asintom&aacute;ticos que se presentan &uacute;nicamente con hallazgos radiol&oacute;gicos, hasta el cuadro de una cistitis muy severa asociada con una alta mortalidad.<sup>1 </sup></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font>     <br> <font size="+3"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="2">    <br> Hay una frecuencia del doble en el sexo femenino. La glicosuria de la <i>diabetes mellitus </i>act&uacute;a como un sustrato de fermentaci&oacute;n para las bacterias formadoras de gas en estos pacientes, y hasta el 50% de ellos son diab&eacute;ticos. Los pacientes con compromiso inmunol&oacute;gico, receptores de trasplante renal, infecciones urinarias a repetici&oacute;n previas, presencia de estasis, vejigas neurog&eacute;nicas y con enfermedad hep&aacute;tica, tambi&eacute;n est&aacute;n en riesgo de contraer la entidad. </font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font>     <br> <font size="+3"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="2">    <br> Se han aislado bacterias anaer&oacute;bicas de <i>Clostridium perfringes </i>y algunas especies de <i>Candida albicans</i>, en especial en pacientes de la tercera edad y con compromiso inmunol&oacute;gico. Tambi&eacute;n se han aislado <i>E. coli </i>y especies productoras de gas de <i>Staphylococus aureus, Proteus mirabilis</i> y <i>Klebsiella pneumonae</i>.<sup>2 </sup></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font>     <br> <font size="+3"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="2">    <br> Generalmente, estos pacientes se presentan con dolor abdominal bajo, fiebre, hematuria, piuria y muy rara vez pneumaturia, y la placa simple de abdomen muestra la presencia de gas con distribuci&oacute;n lineal de burbujas m&uacute;ltiples.     <br>     <br> </font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font></font><font  size="+3"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="+1"><font size="+1"><font size="+1"><font  size="+1"><font size="2">En casos severos, los s&iacute;ntomas sugieren patolog&iacute;a abdominal o p&eacute;lvica aguda, con desprendimiento de la mucosa vesical, gangrena y s&iacute;ntomas de sepsis, que requieren urgente tratamiento quir&uacute;rgico con antibi&oacute;ticos.
