Tatiana Moya<A HREF="#*">*</A></FONT></FONT></B></CENTER> <FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1><I>&nbsp;&nbsp;</I></FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Introducci&oacute;n</FONT></FONT></B>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los miembros del g&eacute;nero <I>Neisseria</I>, son bacterias Gram negativo, agrupadas en pares, oxidasa positivo, catalasa positivo, producen acidez a partir de la glucosa y reducen los nitritos(<A HREF="#9">9</A>,<A HREF="#10">10</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>De este grupo, se han separado los g&eacute;neros <I>Branhamella, Moraxella</I> y <I>Acinetobacter</I>, todos ellos organismos que en el pasado fueron clasificados dentro del g&eacute;nero <I>Neisseria</I>(<A HREF="#9">9</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Las dos especies m&aacute;s conocidas de este g&eacute;nero son <I>Neisseria gonorrhoeae</I> y <I>Neisseria meningitidis</I>. Esta &uacute;ltima, puede ser encontrada colonizando el tracto respiratorio superior de los humanos, pero adem&aacute;s, es una importante causa de enfermedades en ellos(<A HREF="#10">10</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Se le encuentra produciendo especialmente infecciones invasivas como meningitis, septicemia y el S&iacute;ndrome de WaterHouse-Frederichsen(<A HREF="#10">10</A>). Com&uacute;nmente se le conoce como meningococo.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Esta es una bacteria encapsulada, por lo que puede ser dividida serol&oacute;gicamente en varios grupos, de los cuales, los serogrupos A,B,C,Y y W135, son los m&aacute;s frecuentemente asociados a infecciones severas en los humanos (<A HREF="#9">9</A>,<A HREF="#10">10</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Se cuenta con una vacuna tetravalente, pero no cubre al serotipo B, para el cual no hay una vacuna confiable(<A HREF="#9">9</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Tres son los factores importantes para el desarrollo de una infecci&oacute;n meningoc&oacute;ccica: la habilidad de <I>Neisseria meningitidis</I> para la colonizaci&oacute;n de la nasofaringe, para lo cual utiliza un pili o fimbria, con gran habilidad de diseminarse sin despertar una respuesta antig&eacute;nica adecuada y, por &uacute;ltimo, con una alta producci&oacute;n de endotoxinas LPS(<A HREF="#10">10</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Otra de las particularidades de este agente, es la producci&oacute;n de epidemias, ya que su diseminaci&oacute;n v&iacute;a a&eacute;rea, lo hace f&aacute;cilmente propagable de persona a persona especialmente en lugares de asinamiento como guarder&iacute;as, escuelas y hospitales(<A HREF="#10">10</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En el a&ntilde;o de 1971 por primera vez en Costa Rica, se detect&oacute; una epidemia de meningitis por <I>Neisseria meningitidis</I> del serogrupo C. Este serogrupo fue resistente a las sulfas (<A HREF="#7">7</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>A partir del a&ntilde;o 1992, en nuestro pa&iacute;s, se ha incrementado el registro de casos con infecciones invasivas por <I>Neisseria meningitidis</I>.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Se detectaron 2 cepas en 1992 (ambas de serogrupo B), 20 en el a&ntilde;o 1993 (12B, 6C, 1A y 1Y), 26 en el a&ntilde;o 1994 (14A, 7B, 5C) y 34 en el a&ntilde;o de 1995 (9A, 3B, 16C, 3D, 3Y) (<A HREF="#5">5</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La detecci&oacute;n de estas infecciones, se ha hecho en las diferentes regiones del pa&iacute;s y tambi&eacute;n en diferentes estratos sociales, por lo que podemos encontrarnos en una etapa pre epid&eacute;mica.</FONT></FONT>     ]]></body>
<body><![CDATA[<BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Materiales y M&eacute;todos</FONT></FONT></B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Todas las cepas aisladas en el pa&iacute;s, tanto en adultos como en ni&ntilde;os, han sido tipeadas en el Laboratorio del Hospital Nacional de Ni&ntilde;os "Dr. Carlos S&aacute;enz Herrera", utilizando antisueros de la Casa Difco (Difco Lab. Michigan, USA).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Las cepas fueron aisladas en agar chocolate suplementado y agar sangre, con atm&oacute;sfera de C02. Cuando las enviaron de otros laboratorios, se recibieron dentro de un plazo corto de tiempo.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Una vez la cepa recibida, se sembr&oacute; en agar chocolate fresco y se incub&oacute; a 35 grados cent&iacute;grados por 18 a 24 horas en atm&oacute;sfera de CO2.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Una vez con la cepa fresca, se repiti&oacute; la tinci&oacute;n de Gram y se realiz&oacute; una prueba de oxidasa. Adem&aacute;s, inmediatamente se busc&oacute; la producci&oacute;n de beta lactamasa, para lo cual se utiliz&oacute; el m&eacute;todo de la cefalosporina (Cefinase, BBL) (<A HREF="#9">9</A>,<A HREF="#12">12</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Luego de estas pruebas confirmatorias se tipific&oacute; la cepa para lo cual se us&oacute; el pool completo de antisueros de la Casa Difco (Difco Lab. Detroit, Michigan, USA) y por &uacute;ltimo, se realiz&oacute; una prueba de sensibilidad siguiendo los lineamientos de la American Society of Microbiolgy (ASM) (<A HREF="#12">12</A>), utilizando para ello los siguientes antibi&oacute;ticos: cefotaxime, rifampicina y trimetoprim sulfamethoxazole.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Para la sensibilidad a la penicilina, se utiliz&oacute; el disco 1ug de oxaciclina (<A HREF="#16">16</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En todos los casos, el medio base utilizado fue agar sangre, preparado con sangre humana al 5% y agar M&uuml;ller-Hinton como base.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En todos los casos, con una excepci&oacute;n, las pruebas de sensibilidad fueron reportadas sensibles a la penicilina; sin embargo, hay una resistencia del 35% a trimetoprim sulfamethoxazole, resistencia que es semejante para cada uno de los diferentes serogrupos(<A HREF="#6">6</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1><B>Reporte del caso.</B></FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>A inicios del mes de junio de 1994, se recibi&oacute; en el servicio de Emergencias M&eacute;dicas del Hospital Nacional de Ni&ntilde;os "Carlos S&aacute;enz Herrera", una ni&ntilde;a de tres a&ntilde;os de edad, de quien se aisl&oacute; <I>Neisseria meningitidis</I> del serogrupo B.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Dicha cepa fue recuperada de un hemocultivo tomado el d&iacute;a de su ingreso. Un cultivo del l&iacute;quido cefalorraqu&iacute;deo fue negativo, a pesar de tener criterios de infecci&oacute;n men&iacute;ngea, ya que la paciente recibi&oacute; antibi&oacute;ticos antes de su ingreso al hospital.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La ni&ntilde;a fue tratada con penicilina s&oacute;dica a las dosis usuales, pero cuatro d&iacute;as despu&eacute;s, se aisla <I>N. meningitidis</I>, del mismo serotipo, de un cat&eacute;ter venoso central, por lo que la cobertura antimicrobiana fue cambiada a cefotaxime, tambi&eacute;n a las dosis usuales.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La cepa de<I> Neisseria meningitidis </I>del serogrupo B, tanto la aislada del hemocultivo como la del cat&eacute;ter, es productora de B-Lactamasa y resistente, seg&uacute;n la prueba de disco, a ampicilina, amoxicilina/&aacute;cido clavul&aacute;nico y trimetoprim sulfamethoxazole pero sensible a azitromicina, rifampicina, cefotaxime y ciprofloxacina.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El disco de 1ug de oxacilina, mostr&oacute; un halo de 10 mm (resistente), con lo cual se confirma la detecci&oacute;n de la producci&oacute;n de B-lactamasa. La CMI de las dos cepas fue de 4,0 ug/ml.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La cepa fue guardada a -70 grados cent&iacute;grados en caldo de Todd-Hewitt con glicerina al 20% (15) y fue descongelada en junio de 1995. Despu&eacute;s de varios pasajes en agar sangre, se repiti&oacute; la prueba de sensibilidad en disco, la detecci&oacute;n de B-lactamasa y la CMI, pero esta vez se utiliz&oacute; la t&eacute;cnica del E-Test (Casa AB Disk) con agar sangre e incubado en las mismas condiciones.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Nuevamente, se demostr&oacute; una resisitencia de 4,0 ug/ml a la penicilina.</FONT></FONT>     ]]></body>
<body><![CDATA[<BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1><B>&nbsp;&nbsp;</B></FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Comentarios</FONT></FONT></B>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El aislamiento de cepas resistentes a la penicilina, tanto de <I>Neisseria meningitidis</I> como de <I>Streptococcus pneumonmiae</I>, es un fen&oacute;meno mundial (<A HREF="#1">1</A>,<A HREF="#2">2</A>,<A HREF="#3">3</A>,<A HREF="#8">8</A>,<A HREF="#11">11</A>,<A HREF="#13">13</A>,<A HREF="#14">14</A>). Es cada vez m&aacute;s frecuente la detecci&oacute;n de estas cepas y Costa Rica no es la excepci&oacute;n.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Otra bacteria que con frecuencia cambia su patr&oacute;n de sensibilidad es <I>Haemophilus</I> <I>influenzae </I>(<A HREF="#6">6</A>). Sin embargo, en nuestro pa&iacute;s, a&uacute;n continuamos teniendo una adecuada sensibilidad de este agente a la ampicilina (<A HREF="#6">6</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El caso que se est&aacute; reportando, constituye el primer aislamiento en Costa Rica de una cepa de <I>Neisseria meningitidis</I> serogrupo B, productora de altos niveles de resistencia a la penicilina.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los valores del punto de quiebra para este agente contra la penicilina son del orden de 0,01 ug/ml y se considera que una cepa es resistente cuando la CMI sobrepasa el nivel de 1,0 ug/ml (<A HREF="#9">9</A>,<A HREF="#12">12</A>,<A HREF="#16">16</A>). La cepa aislada en este caso presenta una resisitencia alta (4,0 ug/ml).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Este es un caso aislado, pero debe servir como una alarma para realizar una mejor vigilancia de la sensibilidad de esta bacteria.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Es importante destacar que la prueba de detecci&oacute;n de la producci&oacute;n de B-lactamasa utilizando Cefinasa, es un m&eacute;todo excelente, pero que deber ser confirmado haciendo la CMI correspondiente, o en su defecto, la prueba de la sensibilidad a la oxacilina con el disco 1,0 ug (<A HREF="#9">9</A>,<A HREF="#12">12</A>,<A HREF="#16">16</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
