Despu&#233;s de la introducci&#243;n de <span class="SpellE">lovastatina</span> en los EEUU en septiembre de 1987<a href="#1"><sup>1</sup></a>, en junio de 1989, un mill&#243;n de pacientes hab&#237;a recibido prescripci&#243;n de este f&#225;rmaco; la era de las <span class="SpellE">estatinas</span> hab&#237;a comenzado. Por supuesto, las compa&#241;&#237;as farmac&#233;uticas resaltaron las ventajas que cada nueva <span class="SpellE">estatina</span> ten&#237;a, en tanto que era necesario promocionar las ventas; sin embargo, era dif&#237;cil probarlo: la guerra de las <span class="SpellE">estatinas</span> hab&#237;a empezado. Algunas batallas se perdieron, como el exceso de casos de <span  class="SpellE">rabdomiolisis</span> con <span class="SpellE">cerivastatina</span> (16-80 veces m&#225;s que las otras <span class="SpellE">estatinas</span>) y su posterior retirada del mercado<a href="#2"><sup>2</sup></a>. Otra batalla que se forja en los EEUU es la prescripci&#243;n de <span  class="SpellE">estatinas</span> sin receta, afortunadamente sin &#233;xito, hasta ahora. La supervisi&#243;n m&#233;dica puede proteger a los pacientes m&#225;s vulnerables, como los de la tercera edad, de posibles problemas graves con estos f&#225;rmacos<a href="#3"><sup>3</sup></a>.<o:p></o:p></span></p>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">A pesar de todo, las <span class="SpellE">estatinas</span> representan una contribuci&#243;n incre&#237;ble para la medicina cardiovascular y un tratamiento imprescindible para el paciente con riesgo de problemas cardiovasculares. En este momento, en los EEUU existen 7 <span  class="SpellE">estatinas</span> disponibles; en orden alfab&#233;tico: <span class="SpellE">atorvastatina</span>, <span class="SpellE">fluvastatina</span>, <span class="SpellE">lovastatina</span>, <span class="SpellE">pitavastatina</span>, <span class="SpellE">pravastatina</span>, <span class="SpellE">rosuvastatina</span>, y <span class="SpellE">simvastatina</span>. La <span class="SpellE">estatina</span> m&#225;s nueva es la <span  class="SpellE">pitavastatina</span> y una cuesti&#243;n clave es si contribuye en algo nuevo o si es m&#225;s de lo mismo. Las compa&#241;&#237;as farmac&#233;uticas <span class="SpellE">Eli</span> Lilly y <span class="SpellE">Kowa</span> recibieron la aprobaci&#243;n de <st1:personname  productid="la Administraci&#243;n Estadounidense" w:st="on">la Administraci&#243;n Estadounidense</st1:personname> de Alimentos y F&#225;rmacos (FDA) para <span class="SpellE">pitavastatina</span> (<span  class="SpellE">Livalo</span>&#174;) en EEUU en agosto de 2009<a href="#4"><sup>4</sup></a>. <o:p></o:p></span></p>     ]]></body>
<body><![CDATA[<p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">Hay mucho que decir acerca de la <span class="SpellE">pitavastatina</span>, sus ventajas y cu&#225;ndo recetarla (<a href="#Cuadro1">cuadro 1</a>). Existe evidencia que las <span class="SpellE">estatinas</span> disminuyen los niveles plasm&#225;ticos de la coenzima Q-10 (<span class="SpellE">CoQ</span>-10), resultado de la inhibici&#243;n de la reductasa de <span class="SpellE">hidroximetilglutaril</span> coenzima A (HMG <span class="SpellE">CoA</span> reductasa)<a href="#5"><sup>5</sup></a>. Esta enzima es clave en la s&#237;ntesis del <span class="SpellE">mevalonato</span>, el compuesto intermediario en la producci&#243;n del colesterol y tambi&#233;n para la s&#237;ntesis de <st1:personname productid="la CoQ-10"  w:st="on">la <span class="SpellE">CoQ</span>-10</st1:personname>, un componente de la cadena mitocondrial<a href="#6"><sup>6</sup></a>. <span  class="SpellE">Lamperti</span> <i>et al</i> evaluaron la concentraci&#243;n de <span class="SpellE">CoQ</span>-10 en los m&#250;sculos de 18 pacientes que recib&#237;an una <span class="SpellE">estatina</span> y que ten&#237;an s&#237;ntomas musculares o que ten&#237;an un aumento de la <span class="SpellE">creat&#237;nquinasa</span> s&#233;rica<a href="#6"><sup>6</sup></a>. Estos investigadores hallaron estructura muscular normal en 14 pacientes, disfunci&#243;n mitocondrial en 2 y cambios <span class="SpellE">miop&#225;ticos</span> en otros 2; comparada con controles, hab&#237;a una diferencia en la concentraci&#243;n de <span class="SpellE">CoQ</span>-10 en los m&#250;sculos de estos pacientes en m&#225;s de 2 desviaciones est&#225;ndar (DE) por debajo de lo normal en 3 pacientes y en m&#225;s de una DE en 7, aunque esa diferencia no fue significativa; no encontraron evidencia de apoptosis, utilizando una prueba especial de transferasa de <span class="SpellE">deoxinucleotidasa</span>. Los autores concluyeron que existe la posibilidad que la <span  class="SpellE">miopat&#237;a</span> asociada con las <span class="SpellE">estatinas</span> sea el resultado de una disminuci&#243;n de la concentraci&#243;n de <span class="SpellE">CoQ</span>-10 muscular, sin evidencia histoqu&#237;mica o bioqu&#237;mica y que posiblemente, pero sin prueba definitiva, tales pacientes deben recibir un suplemento de <span class="SpellE">CoQ</span>-10 para aumentar sus niveles<a href="#6"><sup>6</sup></a>.    <br> </span></p>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">    <br> </span></p>     <div style="text-align: center;"><span  style="font-size: 10pt; font-family: Verdana;"><a name="Cuadro1"></a><img  alt="Cuadro1" src="/img/revistas/rcc/v13n2/a03i1.jpg"  style="width: 407px; height: 165px;">&nbsp;<o:p></o:p></span></div>     <p style="text-align: justify;" class="MsoNormal"><span class="SpellE"><span  style="font-size: 10pt; font-family: Verdana;">Kawashiri</span></span><span  style="font-size: 10pt; font-family: Verdana;"> <i>et al</i> compararon los efectos de 4 <span class="SpellE">mg</span> de <span class="SpellE">pitavastatina</span> con 20 <span  class="SpellE">mg</span> de <span class="SpellE">atorvastatina</span> en 19 pacientes japoneses con hipercolesterolemia familiar heterocig&#243;tica sobre los niveles de <span  class="SpellE">CoQ</span>-10<a href="#7"><sup>7</sup></a>. La <span  class="SpellE">atorvastatina</span> y la <span class="SpellE">pitavastatina</span> disminuyeron de manera similar el nivel de la lipoprote&#237;na de baja densidad (LDL): 40.7% y 42.8% respectivamente; sin embargo, la <span class="SpellE">atorvastatina</span> disminuy&#243; el nivel plasm&#225;tico de <span class="SpellE">CoQ</span>-10 en 26.1% (p=0.0007) y la <span class="SpellE">pitavastatina</span> solamente un 7.7% (p=0.39). En este estudio, no <span class="SpellE">exisitieron</span> eventos adversos ni anormalidades enzim&#225;ticas en el h&#237;gado o m&#250;sculos. Por supuesto, es necesario acumular m&#225;s informaci&#243;n para determinar el significado de los cambios en los niveles de <span class="SpellE">CoQ</span>-10 para relacionarlos a la seguridad a largo plazo de la <span class="SpellE">pitavastatina</span>; sin embargo, los resultados pueden ser prometedores. <span class="SpellE">Simvastatina</span>, <span class="SpellE">lovastatina</span> y <span class="SpellE">atorvastatina</span> pasan en su metabolismo por el citocromo P450 (CYP) 3A4 y sus inhibidores pueden aumentar las concentraciones de esas <span class="SpellE">estatinas</span>. Por el contrario, la <span class="SpellE">pravastatina</span>, la <span  class="SpellE">rosuvastatina</span> y la <span class="SpellE">pitavastatina</span> que son <span  class="SpellE">hidrof&#237;licas</span>, no dependen del CYP y por tanto no son susceptibles a ninguna inhibici&#243;n del CYP<a href="#9"><sup>9</sup></a> porque su dependencia con &#233;l es m&#237;nima<sup><a href="#10">10</a>,<a href="#11">11</a></sup>. La <span class="SpellE">fluvastatina</span> utiliza el CYP 2C9 para su metabolismo.<o:p></o:p></span></p>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">Hay otros efectos de la <span class="SpellE">pitavastatina</span> a los cuales debemos prestar atenci&#243;n. La enfermedad renal cr&#243;nica tiene alta posibilidad de progresar a la falla renal y tambi&#233;n tiene una fuerte asociaci&#243;n con la enfermedad cardiovascular. El estudio <span  class="SpellE"><i>Livalo</i></span><i> <span class="SpellE">Effectiveness</span> <span class="SpellE">and</span> <span class="SpellE">Safety</span></i> (LIVES) evalu&#243; el efecto de la <span class="SpellE">pitavastatina</span> en la tasa de filtraci&#243;n glomerular (<span class="SpellE">eGFR</span>, en siglas en ingl&#233;s) en 3,119 pacientes que tomaban pitavastatina<a href="#12"><sup>12</sup></a>. Despu&#233;s de 104 semanas, se produjo un aumento en la <span  class="SpellE">eGFR</span> de 5.6 ml/<span class="SpellE">min</span>/1.73 m<sup>2</sup> (P &lt; 0.001), sugiriendo un efecto beneficioso de la <span class="SpellE">estatina</span> a este nivel. El estudio LIVES tambi&#233;n evalu&#243; el efecto de <span  class="SpellE">pitavastatina</span> en la lipoprote&#237;na de alta densidad (HDL), especialmente en los pacientes con HDL baja (menor de 40 <span  class="SpellE">mg</span>/<span class="SpellE">dl</span>); <st1:personname  productid="la HDL" w:st="on">la HDL</st1:personname> aument&#243; un 14.0% a las 12 semanas y un 24.9% a las 104 semanas; los autores tambi&#233;n reportaron dicho aumento despu&#233;s haberse utilizado otra estatina<a  href="#13"><sup>13</sup></a>. Comparada con la informaci&#243;n existente con otras <span class="SpellE">estatinas</span>, estos resultados parecen significativos, pero necesitamos m&#225;s estudios que confirmen que la <span class="SpellE">pitavastatina</span> aumenta <st1:personname productid="la HDL" w:st="on">la HDL</st1:personname> en 24.9%. Por comparaci&#243;n directa, el efecto de la <span class="SpellE">atorvastatina</span> sobre <st1:personname productid="la HDL" w:st="on">la HDL</st1:personname> es menor, comparado con la pitavastatina<a href="#14"><sup>14</sup></a>; de hecho, despu&#233;s de 6 meses de tratamiento con <span class="SpellE">atorvastatina</span>, <st1:personname  productid="la HDL" w:st="on">la HDL</st1:personname> puede disminuir, pero no con la pitavastatina<a href="#15"><sup>15</sup></a>. En general, el efecto de las <span class="SpellE">estatinas</span> sobre <st1:personname productid="la HDL" w:st="on">la HDL</st1:personname> es moderado y con la <span class="SpellE">atorvastatina</span>, el nivel de HDL puede tener una relaci&#243;n inversa en relaci&#243;n a la dosis, contrario a otras <span  class="SpellE">estatinas</span> como <span class="SpellE">rosuvastatina</span> y simvastatina<a href="#16"><sup>16</sup></a>. Con relaci&#243;n a la prote&#237;na C-reactiva de alta sensibilidad, parece que la <span class="SpellE">pitavastatina</span> es similar a otras <span class="SpellE">estatinas</span> y tiene un efecto significativo en disminuirla<a href="#17"><sup>17</sup></a>. <o:p></o:p></span></p>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">En conclusi&#243;n, la <span class="SpellE">pitavastatina</span> es la s&#233;ptima <span class="SpellE">estatina</span> disponible.<o:p></o:p></span></p>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">Para el paciente de alto riesgo cardiovascular, que sea problem&#225;tico y que no tolere otra <span class="SpellE">estatina</span>, la <span class="SpellE">pitavastatina</span> ofrece caracter&#237;sticas &#250;nicas. Estas pueden resultar en una tolerancia no observada con las otras <span class="SpellE">estatinas</span> y por lo tanto, un beneficio para la terapia farmacol&#243;gica cardiovascular. Este es el tipo de beneficio que un producto nuevo de una misma clase debe ofrecer, en lugar de la inundaci&#243;n del mercado con productos que para competir resultan en guerras de mercadeo que no traen beneficio alguno. <o:p></o:p></span></p>     <p class="MsoNormal"><b style=""><span  style="font-size: 11pt; font-family: Verdana;"><o:p> </o:p> <hr style="width: 100%; height: 2px;"></span></b></p>     ]]></body>
