<body><![CDATA[<p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">Las razones estandarizadas, aun cuando son muy utilizadas en el an&#225;lisis geogr&#225;fico de eventos en salud, tienen algunas limitaciones. En primer lugar, no tiene en cuenta las diferencias en el tama&#241;o de poblaci&#243;n entre &#225;reas geogr&#225;ficas y, por lo tanto, cuando el tama&#241;o de las poblaciones son muy peque&#241;as las razones se vuelven inestables, pasando de cero a razones muy altas, cuando se pasa de ninguno a unos pocos casos de la enfermedad. El otro problema es que los casos observados generalmente var&#237;an m&#225;s que los esperados, produci&#233;ndose lo que se denomina extra-variabilidad. Un an&#225;lisis detallado de los problemas del uso directo de la raz&#243;n estandarizada sin ajustar se presenta en <span class="SpellE">Cressie</span>-Chan y Clayton-<span  class="SpellE">Kaldor</span> <sup>(<a href="#1">1, 2</a>)</sup>. <o:p></o:p></span></p>     <div style="text-align: justify;"></div>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">Para solventar estos problemas, se estiman las razones estandarizadas suavizadas. Se han propuesto varias alternativas para suavizar las razones estandarizadas y reducir la <span class="SpellE">extravariaci&#243;n</span>. Seg&#250;n la literatura <sup>(<a href="#2">2-6</a>)</sup> el m&#233;todo de estimaci&#243;n denominado <span class="SpellE">bayesiano</span> emp&#237;rico y los modelos jer&#225;rquicos <span class="SpellE">bayesianos</span> completos son los que proporcionan los resultados m&#225;s consistentes respecto a otras alternativas. No obstante, los modelos completamente <span class="SpellE">bayesianos</span> son preferidos porque son capaces de modelizar la dependencia (<span class="SpellE">extravariabilidad</span>) espacial <sup>(<a href="#7">7</a>)</sup>. <o:p></o:p></span></p>     <div style="text-align: justify;"></div>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">Otro problema en el an&#225;lisis geogr&#225;fico de eventos en salud es que la mayor&#237;a de aplicaciones emp&#237;ricas utilizan <span class="SpellE">elan&#225;lisis</span> puramente espacial dejando de lado la dimensi&#243;n temporal. El resultado es un patr&#243;n promedio del riesgo a lo largo del periodo analizado y como consecuencia los resultados no aportan informaci&#243;n sobre la variabilidad que sufre el riesgo a lo largo del tiempo y no se sabe si un riesgo alto es sostenido o ha cambiado con el tiempo <sup>(<a href="#8">8</a>)</sup>. <o:p></o:p></span></p>     <div style="text-align: justify;"></div>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">El uso de periodos de tiempo agregados puede producir un sesgo en las estimaciones de los riesgos relativos de manera que el exceso de eventos observados en algunas &#225;reas geogr&#225;ficas podr&#237;a ser s&#243;lo el reflejo de una situaci&#243;n pasada que contin&#250;a visualiz&#225;ndose debido a la agregaci&#243;n de informaci&#243;n <sup>(<a href="#9">9</a>)</sup>. <o:p></o:p></span></p>     <div style="text-align: justify;"></div>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">Estos problemas se resuelven con la utilizaci&#243;n de modelos <span  class="SpellE">bayesianos</span> jer&#225;rquicos espaciotemporales ya que permiten mostrar la distribuci&#243;n geogr&#225;fica de riesgos y c&#243;mo evolucionan &#233;stos a lo largo del tiempo. Por este motivo, el uso de modelos <span  class="SpellE">bayesianos</span> jer&#225;rquicos espacio-temporales se han generalizado en la cartograf&#237;a de enfermedades <sup>(<a href="#10">10-13</a>)</sup>. <o:p></o:p></span></p>     <div style="text-align: justify;"></div>     ]]></body>
<body><![CDATA[<p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">Los modelos <span class="SpellE">bayesianos</span> jer&#225;rquicos espaciotemporales permiten capturar correctamente elementos de dependencia entre &#225;reas de salud y la posible relaci&#243;n de los datos en el tiempo que no se pueden modelar en la estad&#237;stica cl&#225;sica. Los resultados de estos modelos nos brindan los insumos para el mapeo de enfermedades y la identificaci&#243;n de posibles patrones geogr&#225;ficos persistentes en el tiempo que son de utilidad para la vigilancia epidemiol&#243;gica y la priorizaci&#243;n de intervenciones en zonas de alto riesgo. <o:p></o:p></span></p>     <div style="text-align: justify;"></div> </div>     <div style="text-align: justify;"></div>     <p style="text-align: justify;" class="MsoNormal"><span  style="font-size: 10pt; font-family: Verdana;">En la literatura <sup>(<a href="#10">10, 11</a>, <a href="#14">14</a>)</sup> existen varias formas de modelizar los componentes espaciales y temporales. Se ha demostrado <sup>(<a href="#15">15</a>)</sup> que incluyen <span class="GramE">factores aleatorios espacial</span> y temporal y un componente que <span  class="SpellE">modeliza</span> la interacci&#243;n espaciotemporal, presentan los mejores resultados, tanto para muestras grandes como peque&#241;as.
