Marlen Campos<A HREF="#*">*</A></FONT></FONT></B></CENTER>      <CENTER><FONT SIZE=-1>&nbsp;</FONT></CENTER> &nbsp;<B><FONT FACE="Arial"><FONT SIZE=-1></FONT></FONT></B>      <P><B><FONT FACE="Arial"><FONT SIZE=-1>Introducci&oacute;n</FONT></FONT></B>      <P><FONT FACE="Arial"><FONT SIZE=-1>Hasta antes del a&ntilde;o 1970 (<A HREF="#5">5</A>), dentro del g&eacute;nero <I>Streptococcus</I> se encontraban algunos organismos que ten&iacute;an caracter&iacute;sticas que los diferenciaban del resto de especies de este grupo.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Ejemplos de especies con estas caracter&iacute;sticas, tenemos al <I>Streptococcus faecalis</I> y al <I>streptococcus faecium</I>. Las diferencias que presentaban se encontraban principalmente en el agrupamiento celular, en varias caracter&iacute;sticas fenot&iacute;picas y principalmente en su patr&oacute;n de sensibilidad, el cual difer&iacute;a completamente, del observado para el resto del g&eacute;nero (<A HREF="#4">4</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Sin embargo, no fue hasta el a&ntilde;o de 1984, cuando Schleifer y Kilpper-Balz (<A HREF="#20">20</A>), usando estudios gen&eacute;ticos, propusieron la creaci&oacute;n de un nuevo g&eacute;nero donde se pudieran agrupar estos agentes y de esta manera se crea el g&eacute;nero <I>Enterococcus.</I></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial"><FONT SIZE=-1>Hasta el momento, 17 especies est&aacute;n constitu&iacute;das en este grupo de microorganismos (<A HREF="#3">3</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Los <I>Enterococcus</I> son cocos Gram positivo, encontrados solos, en pares o formando cadenas y eventualmente pueden tener una morfolog&iacute;a cocobacilar.</FONT></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Son anaerobios facultativos y tienen un rango &oacute;ptimo de crecimiento entre 30 y 35</FONT><FONT FACE=Symbol>&deg;</FONT><FONT FACE="Arial"> C. Todas las especies crecen en caldos con 6,5% de NaCl e hidrolizan la esculina en la presencia de sales biliares. Son organismos catalasa negativa, pero en algunas oportunidades pueden ser catalasa positivo y el &aacute;cido l&aacute;ctico es el producto final de la fermentaci&oacute;n de la glucosa (<A HREF="#3">3</A>).</FONT></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Muchos de estos agentes pueden producir el ant&iacute;geno D, usado en la clasificaci&oacute;n de los <I>Streptococcus</I> sp. </FONT><FONT FACE=Symbol>b</FONT><FONT FACE="Arial"> -hemol&iacute;ticos (<A HREF="#3">3</A>). El rango del contenido de Guanina m&aacute;s Citosina para este grupo es de 37 a 45 mol % (<A HREF="#3">3</A>).</FONT></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">En t&eacute;rminos generales los <I>Enterococcus </I>sp. son cocos Gram positivo, catalasa negativo, anaerobios facultativos, no producen gas a partir de la fermentaci&oacute;n de la glucosa, son pirolidonil </FONT><FONT FACE=Symbol>b</FONT><FONT FACE="Arial"> -naftilamida positivos, leucemia amino peptidasa positivos, bilis esculina positivos, crecen en caldos con NaCl al 6,5% y pueden ser alfa, beta o gamma hemol&iacute;ticos (<A HREF="#3">3</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Cada vez se hace m&aacute;s necesaria una confirmaci&oacute;n fidedigna y una mejor clasificaci&oacute;n de los componentes de este g&eacute;nero, para lo cual se pueden utilizar los sistemas automatizados para la identificaci&oacute;n bacteriana como el Sistema Vitek de la caso BioMerieux o el Sistema MicroScan de la casa Dade. Adem&aacute;s pueden usarse sondas gen&eacute;ticas (accuProbe Enterococcus Test), prove&iacute;dos por la casa Gen Probe Inc. En San Diego California (<A HREF="#3">3</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Los <I>Enterococcus</I> sp., se distinguen por su capacidad de sobrevivir en el medio ambiente, por lo que pueden ser encontrados en el suelo, los alimentos, agua, animales dom&eacute;sticos, aves e insectos. Adem&aacute;s se les puede encontrar formando parte de la flora normal intestinal y del tracto genito urinario de los seres humanos (<A HREF="#3">3</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Como causa de patolog&iacute;a, los enterococos han sido implicados en una gran variedad de cuadros cl&iacute;nicos, pero en especial, ellos est&aacute;n relacionados con infecciones nosocomiales, principalmente aquellas que se relacionan con el tracto urinario (<A HREF="#14">14</A>,<A HREF="#16">16</A>). Otros tipos de infecciones, en las cuales los enterococos son importantes son las infecciones intra abdominales y p&eacute;lvicas (<A HREF="#19">19</A>), las bacteremias y septicemias (<A HREF="#9">9</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Al <I>Enterococcus faecalis</I>, se le atribuye ser el agente causal de entre un 5 a 20% de todos los casos de endocarditis bacteriana (<A HREF="#12">12</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>En general, podemos concluir, que los <I>Enterococcus sp., </I>son organismos ampliamente distribuidos en la naturaleza, que es un importante pat&oacute;geno productor de infecciones principalmente intra hospitalarias, donde se le a&iacute;sla de infecciones urinarias, septicemias, endocarditis, infecciones abdominales pos cirug&iacute;as e infecciones de heridas (<A HREF="#9">9</A>, <A HREF="#12">12</A>, <A HREF="#14">14</A>, <A HREF="#16">16</A>, <A HREF="#19">19</A>).</FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial"><FONT SIZE=-1>Los enterococos crecen bien en los medios de cultivo tradicionales, donde el agar sangre con sangre al 5% y una base de tripticasa soya puede ser un buen medio para el aislamiento primario (<A HREF="#4">4</A>). Crecen mejor en una atm&oacute;sfera enriquecida con CO<SUB>2</SUB>, pero esto no es un requisito indispensable para su aislamiento (<A HREF="#3">3</A>,<A HREF="#4">4</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Intr&iacute;secamente los enterococos son resistentes a muchos antibi&oacute;ticos, como por ejemplo, la penicilina.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Este fue uno de los hechos m&aacute;s relevantes al crear el nuevo g&eacute;nero y sacar, grupos de bacteria resistentes a los antibi&oacute;ticos de un grupo que comprende bacterias con amplia sensibilidad a los diferentes antibi&oacute;ticos. Al ser resistentes a los antibi&oacute;ticos tradicionales, se hace necesario el uso de antibi&oacute;ticos con mayor efecto como es el caso de la vancomicia, sin embargo, muchas especies del g&eacute;nero <I>Enterococcus</I> tienen resistencia intr&iacute;nseca a este antibi&oacute;tico, como es el caso de <I>Enterococcus gallinarum, Enterococcus casseliflavus </I>y <I>Enterococcus flavescens</I> (<A HREF="#1">1</A>,<A HREF="#18">18</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Todo esto hace que las infecciones por enterococos sean dif&iacute;ciles de tratar y que generalmente necesiten del uso de antibi&oacute;ticos sin&eacute;rgicos como es el caso de la vancomicina y un aminogluc&oacute;sido (amikacina o gentamicina).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>A partir del a&ntilde;o de 1968, emergieron, en el &aacute;mbito mundial cepas de <I>Enterococcus faecalis, Enterococcus faecium </I>y <I>Enterococcus avium</I> resistentes a la vancomicina (<A HREF="#23">23</A>). Dichos reportes se iniciaron en los pa&iacute;ses europeos y en 1989, se detect&oacute; el primer aislamiento de este tipo en los EE.UU (<A HREF="#2">2</A>,<A HREF="#3">3</A>,<A HREF="#14">14</A>,<A HREF="#16">16</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>En Am&eacute;rica Latina, el primer reporte se dio hasta el a&ntilde;o de 1998 y fue reportado por Mar&iacute;n et al. en Argentina (<A HREF="#10">10</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Este hecho, nos presenta un panorama a&uacute;n m&aacute;s complicado, donde las opciones terap&eacute;uticas para un paciente infectado con un enterococo resistente a la vancomicina, se ven muy reducidas, con un consiguiente aumento en la mortalidad de este tipo de pacientes (<A HREF="#14">14</A>,<A HREF="#16">16</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>La vancomicina es un antibi&oacute;tico bactericida, obtenido de la fermentaci&oacute;n del <I>Streptomyces orientalis</I> e inhibe la s&iacute;ntesis del peptodoglican de la pared celular bacteriana al formar un complejo con la porci&oacute;n D-alanil-D-alanina de los precursores de la pared celular (<A HREF="#17">17</A>). Es muy activo contra las cepas de <I>Streptococcus</I> sp<I>., Corynebacterium jeikeum, Bacillus</I> sp.,<I> Clostridium difficile, Flavobacterium meningosepticum</I> y funciona como un agente bacteriost&aacute;tico al actuar contra las cepas de <I>Enterococcus </I>sp. (<A HREF="#24">24</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>La vancomicina no es activa contra los organismos Gram negativo, las mycobacterias y los hongos y las cepas de <I>Leuconostoc</I> sp. y <I>Pediococcus </I>sp., tambi&eacute;n tienen resistencia intr&iacute;seca (<A HREF="#24">24</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Como se dijo anteriormente, desde el a&ntilde;o 1985, se han realizado reportes de aislamientos cl&iacute;nicos de <I>Enterococcus faecalis</I> (<A HREF="#13">13</A>), <I>Enterococcus faecium</I> (<A HREF="#8">8</A>), <I>Enterococcus gallinarum</I> (<A HREF="#7">7</A>) y <I>Staphylococcus</I> sp. coagulasa negativo (<A HREF="#21">21</A>), resistentes a la vancomicina.</FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial"><FONT SIZE=-1>Las bacterias Gram negativo son resistentes a la vancomicina, porque &eacute;sta no puede atravesar la membrana externa y no lo hace por su alto peso molecular, la rigidez de su estructura molecular y su alta hidrofobicidad (<A HREF="#24">24</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Para los organismos Gram positivo, se han escrito varias formas o maneras de crear resistencia a la vancomicina. Uno de ellos es el desarrollo de un fenotipo VanA que confiere resistencia conjunta a la vancomicina y a la teicoplamina con concentraciones m&iacute;nimas inhibitorias (MICs) mayores o iguales a 64 ug de droga activa. Un segundo mecanismo es el desarrollo de otro fenotipo VanB, que confiere una pobre resistencia al antibi&oacute;tico , con MICs que van desde 16 a 512 ug de droga activa. Otro es el fenotipo VanC el cual es constitutivo y de poca importancia cl&iacute;nica, se presenta asociado a <I>Enterococcus gallinarum y Enterococcus casseliflavus</I> y dichas cepas presentan MICs entre 2 y 32 ug de droga activa (<A HREF="#6">6</A>,<A HREF="#22">22</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Dentro de los efectos adversos a la vancomicina que pueden presentar las personas tratadas con este medicamento est&aacute;n la fiebre, los escalofr&iacute;os y la flebitis en el sitio de la infusi&oacute;n. En un 5% de los pacientes tratados, se puede presentar un cuadro de hipersensibilidad conocido como el S&iacute;ndrome del Hombre Rojo, as&iacute; como se puede inducir ototoxicidad, nefrotoxicidad y hepatotoxicidad (<A HREF="#6">6</A>,<A HREF="#24">24</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Dada la importancia de la posible aparici&oacute;n de cepas de <I>Enterococcus</I> sp. vancomicina resistentes y existiendo en el laboratorio cl&iacute;nico de Hospital Nacional de Ni&ntilde;os una vigilancia epidemiol&oacute;gica del patr&oacute;n semestral de las bacterias aisladas, surgi&oacute;, sobre la base de la revisi&oacute;n de estos datos, la idea de realizar una b&uacute;squeda sistem&aacute;tica de ni&ntilde;os portadores de cepas de <I>Enterococcus </I>sp. intermedios o resistentes a la vancomicina.</FONT></FONT>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>Material y M&eacute;todos</FONT></FONT></B>      <P><FONT FACE="Arial"><FONT SIZE=-1>En mayo de 1997, se inici&oacute; la recolecci&oacute;n de las muestras de heces.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Inicialmente se estudiaron ni&ntilde;os ingresados a la Unidad de Cuidados Intensivos (UCI) y se buscaron pacientes que hubieran recibido antibi&oacute;ticos.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Se inici&oacute; con la UCI, porque los ni&ntilde;os ingresados all&iacute;, reciben altas dosis de antibi&oacute;ticos, dada la gravedad de su condici&oacute;n cl&iacute;nica.</FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial"><FONT SIZE=-1>Durante un periodo de 8 meses, se estudiaron 110 ni&ntilde;os, cuatro en mayo, 9 en setiembre, 41 en octubre, 32 en noviembre y 28 en diciembre.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Las muestras de heces se obtuvieron por hisopado rectal y se les recolectaron a los pacientes con, al menos, 2 d&iacute;as de permanencia en la UCI.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Las muestras de heces, recolectadas, fueron llevadas al Laboratorio y una vez all&iacute;, se inocularon en dos tipos diferentes de medio de cultivo.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>El primero de ellos fue un agar sangre al 7%, con sangre de banco de sangre y una base de Mueller Hinton. El segundo medio de cultivo empleado fue el mismo, pero se le adicion&oacute; vancomicina (Fujisawa USA Inc) a una concentraci&oacute;n de 6 ug por mililitro de medio de cultivo (<A HREF="#22">22</A>).</FONT></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Ambos medios de cultivo, se incubaron por 48 horas a 35</FONT><FONT FACE=Symbol>&deg;</FONT><FONT FACE="Arial"> C en una atm&oacute;sfera rica en CO2 y con revisi&oacute;n a las 24 horas.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Aquellas colonias no hemol&iacute;ticas o alfa hemol&iacute;ticas, que fueran cocos Gram positivo y catalasa negativa, se rayaron sobre una agar sangre fresco y se incubaron por un periodo de 16 a 18 horas.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Pasado este per&iacute;odo de incubaci&oacute;n, se mont&oacute; una identificaci&oacute;n y una prueba de sensibilidad, para la cual se utiliz&oacute; el sistema automatizado Vitek de la casa BioMerieux, utiliz&aacute;ndose para este efecto, tarjetas GPI para la identificaci&oacute;n y tarjetas TA para la prueba de sensibilidad. En todo momento se emple&oacute; el protocolo recomendado por la casa BioMerieux.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Los est&aacute;ndares internacionales para la t&eacute;cnica de Bauer y Kirby, dicen que una cepa de <I>Enterococcus</I> sp., con un halo de inhibici&oacute;n menor o igual a 14 mm debe ser considerada resistente, intermedia si su halo est&aacute; entre 15 y 16 mm y sensible si &eacute;ste es mayor de 17 mm, todo esto para un disco normal con 30 ug de droga activa. Para la M&iacute;nima Concentraci&oacute;n Inhibitoria (MIC), los datos aceptados son los siguientes: sensible: menor o igual a 4 ug, intermedio: 8 a 16 ug y resistente si la cepa presenta una Mic mayor o igual a 32 ug de droga activa (<A HREF="#22">22</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Luego, durante los meses de abril y mayo del a&ntilde;o de 1998, se realiz&oacute; un tamizaje diferente.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>En este segundo tamizaje, se estudiaron las muestras de heces de 190 ni&ntilde;os, todos menores de 2 meses, que no estuviesen recibiendo antibi&oacute;ticos y con una estancia hospitalaria menor a las 12 horas al momento de la recolecci&oacute;n de la muestra.</FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial"><FONT SIZE=-1>En el laboratorio, se emplearon los mismos tipos de medios de cultivo, la misma t&eacute;cnica de aislamiento e identificaci&oacute;n y se siguieron los mismos par&aacute;metros para la interpretaci&oacute;n de los resultados obtenidos.</FONT></FONT>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>Resultados</FONT></FONT></B>      <P><FONT FACE="Arial"><FONT SIZE=-1>En la primera parte del estudio, se estudiaron 110 pacientes y de ellos se aislaron 18 cepas de <I>Enterococcus</I> sp. , para un 15,4% de aislamiento, de los cuales 2 fueron intermedios (MIC entre 8 y 16 ug), 2 fueron resistentes (MIC mayor de 32 ug) y 14 sensible con MICs menores o iguales a 2 ug de vancomicina.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>La distribuci&oacute;n de las cepas aisladas fue Ia siguiente: <I>Enterococcus faecalis</I> 9 cepas, <I>Enterococcus faecium</I> 4 cepas, <I>Enterococcus avium</I> 2 cepas, <I>Enterococcus gallinarum, Streptococcus bovis, Sterptococcus sanguineus</I> una cepa cada uno. <body><![CDATA[<BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En este grupo de estudio, tampoco se encontr&oacute; una relaci&oacute;n con la procedencia de los ni&ntilde;os y el hallazgo de cepas intermedias o resistentes a la vancomicina y es importante recordar que todos eran menores de 2 meses.</FONT></FONT>     <CENTER><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B></CENTER> <B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Conclusiones</FONT></FONT></B>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El presente estudio, es el primero realizado en Costa Rica y posiblemente en Centro Am&eacute;rica, donde se demuestre la presencia de cepas de especies del g&eacute;nero<I> Enterococcus</I> con resistencia a la vancomicina.</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Esta investigaci&oacute;n est&aacute; dividida en dos estudios. En el primero de ellos, se emplean heces de ni&ntilde;os internados en una unidad de cuidado intensivo, con una muy variada edad y que reciben diferentes tipos de antibi&oacute;ticos. En la segunda parte, se estudian ni&ntilde;os de poca edad, provenientes de la comunidad, que no reciben antibi&oacute;ticos en el hospital y con muy poca estancia hospitalaria.</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Estos &uacute;ltimos pacientes, fueron escogidos, porque no han tenido una historia previa de terapias antimicrobianas y menos por tiempos prolongados.</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En ambos grupos, los ni&ntilde;os reclutados provienen de lugares muy diversos de la topograf&iacute;a nacional y no fue posible encontrar una relaci&oacute;n epidemiol&oacute;gica entre ellos. En especial, los ni&ntilde;os portadores de cepas de <I>Enterococcus</I> sp. resistentes o intermedios a la vancomicina, no presentaron ning&uacute;n aspecto epidemiol&oacute;gico que pudiese relacionarlos y son ni&ntilde;os que provienen tanto de &aacute;reas rurales como citadinas. As&iacute; como la procedencia, ni la edad ni el sexo fue un factor importante entre ellos.</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La emergencia de cepas de <I>Enterococcus</I> sp. resistentes a la vancomicina viene de unos 10 a&ntilde;os atr&aacute;s y se inici&oacute; principalmente en los pa&iacute;ses europeos. Los cient&iacute;ficos europeos, han tratado de encontrar una explicaci&oacute;n a este fen&oacute;meno, pero sin resultados concluyentes. Ellos atribuyen esta emergencia al uso indiscriminado de antibi&oacute;ticos en la alimentaci&oacute;n animal, sin embargo aun hay muchos interrogantes al respecto (<A HREF="#11">11</A>).</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En este estudio, no hemos logrado explicar el origen, en nuestro entorno, de estas cepas.</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Lo importante es que est&aacute;n y que es necesario tomar medidas que nos permitan controlar este problema.</FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Posiblemente, la medida m&aacute;s importante es racionalizar el uso de vancomicina; a todo nivel.</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Adem&aacute;s, es importante resaltar que, con excepci&oacute;n de las cepas que poseen resistencia intr&iacute;nseca, el desarrollo de esta resistencia es mediada gen&eacute;ticamente y que esta resistencia puede ser donada. As&iacute; pues, los fenotipos VanA, VanB y VanC podr&iacute;an, por recombinaci&oacute;n bacteriana, ser encontrados en cepas de <I>Staphylococcus aureus o Staphylococcus </I>sp. coagulasa negativo, lo cual ser&iacute;a de gran perjuicio para la salud de los costarricenses y producir&iacute;a un aumento en los costos hospitalarios.</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Otro aspecto importante de destacar es la identificaci&oacute;n de cepas como <I>Enterococcus gallinarum </I>y <I>Enterococcus casseliflavus</I>, los cuales son ampliamente reconocidos como portadores de fenotipos como el VanC, los que les confiere resistencia intr&iacute;nseca, resistencia que fue determinada y claramente definida por nuestro sistema automatizado a&uacute;n cuando esta resistencia es de bajo nivel.</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El incremento en el uso de la vancomicina se ha presentado en todo el mundo y nuestro pa&iacute;s no es la excepci&oacute;n. Las principales razones para este incremento est&aacute;n en el aumento en la incidencia de <I>Staphylococcus</I> meticilina resistente, el gran auge en las infecciones derivadas de la presencia de cat&eacute;teres, la incidencia de infecciones por <I>Clostridium difficile </I>y el uso inapropiado de esta droga (<A HREF="#11">11</A>).</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Como recomendaciones para impedir la diseminaci&oacute;n de los <I>Enterococcus</I> vancomicina resistentes a escala hospitalaria, se mencionan varias cosas, entre ellas est&aacute; la promoci&oacute;n del lavado de manos y la reducci&oacute;n dr&aacute;stica del uso de los glicop&eacute;ptidos (<A HREF="#11">11</A>).</FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Las opciones de tratamiento para infecciones por<I> Enterococcus</I> resistentes a la vancomicina son escasas y la gran mayor&iacute;a de las personas tratadas en estos casos, han recibido tratamientos emp&iacute;ricos. Se ha utilizado la ampicilina, el sinergismo con la gentamicina y el uso de nitrofuranto&iacute;na para infecciones urinarias (<A HREF="#15">15</A>).</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Resumen</FONT></FONT></B>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Se hace referencia a la creaci&oacute;n del g&eacute;nero <I>Enterococcus</I> y sus especies importantes en patolog&iacute;a humana as&iacute; como su comportamiento ante la vancomicina y otros antimicrobianos.</FONT></FONT>      ]]></body>
