Marlen Campos<A HREF="#*">*</A>.</FONT></FONT></B></CENTER> <FONT SIZE=-1>&nbsp;</FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1>&nbsp;</FONT>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>Introducci&oacute;n</FONT></FONT></B><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Los organismos clasificados dentro del g&eacute;nero <I>Prototheca</I>, son algas sin clorofila, que se encuentran ampliamente distribuidos en la naturaleza y que raramente son causa de infecciones en humanos. Por el contrario, pueden ser contaminantes de medios de cultivo dada su amplia distribuci&oacute;n antes mencionada, al ser sapr&oacute;fitos comunes en el suelo y detritos org&aacute;nicos y en ambientes acu&aacute;ticos (<A HREF="#2">2</A>,<A HREF="#3">3</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Este g&eacute;nero presenta dos especies, <I>P. wickerhamii y P. zopfii.</I> Esta &uacute;ltima est&aacute; relacionada con infecciones en animales, mientras que <I>P. wickerhamii</I> lo est&aacute; en casos humanos (<A HREF="#3">3</A>). En estas circunstancias, <I>P. wickerhamii</I> se ha encontrado produciendo infecciones oportunistas en piel y tejidos subcut&aacute;neos, en especial en pacientes sid&oacute;ticos, en pacientes diab&eacute;ticos o inmunosupresos en general (<A HREF="#1">1</A>,<A HREF="#2">2</A>,<A HREF="#3">3</A>,<A HREF="#4">4</A>). Generalmente, su v&iacute;a de ingreso es traum&aacute;tica(<A HREF="#2">2</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Las colonias de la <I>Prototheca wickerhamii</I> son blancas o cremosas y asemejan mucho el crecimiento de una levadura. Son inhibidos por la cicloheximida por lo que no crecen en el agar micosel. La temperatura &oacute;ptima de este agente es de 30</FONT><FONT FACE=Symbol>&deg;</FONT><FONT FACE="Arial"> C, pero crece bien a 35</FONT><FONT FACE=Symbol>&deg;</FONT><FONT FACE="Arial"> C (<A HREF="#3">3</A>). En agar sangre y agar chocolate, sus colonias son un poco m&aacute;s mucosas, pero conforme el cultivo se hace viejo, ellas se van secando hasta adquirir la apariencia de una colonia seca y el color blanco se acent&uacute;a.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">La tinci&oacute;n de Gram es un poco dif&iacute;cil de realizar, ya que este agente no se ti&ntilde;e bien. Es Gram positivo, pero se ti&ntilde;e irregularmente, dando una apariencia de pelota de golf; adem&aacute;s, microsc&oacute;picamente, las c&eacute;lulas var&iacute;an de tama&ntilde;o y forma.</FONT></FONT><FONT SIZE=-1></FONT>      ]]></body>
<body><![CDATA[<P><FONT SIZE=-1><FONT FACE="Arial">Este grupo de microorganismos se reproduce asexualmente formando esporangiosporas que provienen de un esporangio (<A HREF="#2">2</A>,<A HREF="#3">3</A>). Esta &uacute;ltima estructura es la que se ti&ntilde;e con Gram, con la apariencia de pelota de golf antes mencionada.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Por &uacute;ltimo son organismos no fermentadores, pero la <I>P. wickerhamii </I>asimila la glucosa, galactosa y trehalosa, mientras que <I>P. zopfii </I>s&oacute;lo asimila la glucosa y la galactosa (<A HREF="#3">3</A>).</FONT></FONT>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>Reporte de un caso cl&iacute;nico</FONT></FONT></B>     <BR><FONT SIZE=-1>&nbsp;</FONT>     <BR><FONT SIZE=-1><FONT FACE="Arial">En la Secci&oacute;n de Bacteriolog&iacute;a del Laboratorio Cl&iacute;nico del Hospital Nacional de Ni&ntilde;os en San Jos&eacute;, Costa Rica, se recibi&oacute; una muestra de L&iacute;quido Cefalorraqu&iacute;deo (LCR) de un ni&ntilde;o reci&eacute;n nacido que presentaba datos cl&iacute;nicos y de laboratorio acordes con una meningitis.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Los platos de agar sangre y agar chocolate utilizados rutinariamente, no mostraron el crecimiento de alg&uacute;n microorganismo. Sin embargo un tubo con tioglicolato, a las 72 horas de incubaci&oacute;n present&oacute; un crecimiento que fue sembrado en platos de agar sangre, agar McConkey y agar manitol sal. Despu&eacute;s de 24 horas de incubaci&oacute;n a 35</FONT><FONT FACE=Symbol>&deg;</FONT><FONT FACE="Arial"> C, se observaron colonias cremosas las cuales al te&ntilde;irse con Gram, mostraron unas estructuras Gram positivas, que se te&ntilde;&iacute;an mal. Como asemejaban colonias de organismos levaduriformes, se decidi&oacute; tratar de identificarlas utilizando el sistema automatizado Vitek de la casa BioMerieux, para lo cual se inocularon 1,8 ml de soluci&oacute;n salina est&eacute;ril hasta lograr una turbidez del est&aacute;ndar 3 de McFarlane, con la cual se inocul&oacute; una tarjeta yeast de la misma casa comercial y se introdujo al sistema. La colonia fue identificada como una <I>Prototheca wickerhamii, </I>con un 99% de certeza.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Inmediatamente, se inform&oacute; al m&eacute;dico tratante y se solicit&oacute; una nueva muestra del LCR de este paciente. Una segunda muestra fue recibida 48 horas despu&eacute;s, la cual fue reportada negativa. En esta segunda oportunidad, el paciente estaba recibiendo fluconazole a las dosis usuales y presentaba una adecuada recuperaci&oacute;n.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Con la idea de encontrar la posible v&iacute;a de ingreso del agente al ni&ntilde;o, se habl&oacute; con la madre, la que refiere que lo estaba alimentando s&oacute;lo con leche materna.</FONT></FONT><FONT SIZE=-1></FONT>      ]]></body>
<body><![CDATA[<P><FONT SIZE=-1><FONT FACE="Arial">Se le solicit&oacute; la colaboraci&oacute;n a la madre y se obtuvieron varios cultivos de ella: concretamente, secreci&oacute;n vaginal, leche materna de ambos senos y adem&aacute;s se recolect&oacute; una muestra de la piel alrededor del pez&oacute;n de cada uno de los pechos.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Las muestras fueron recolectadas e inoculadas en agar sangre, agar chocolate y agar subouraud glucosado e incubadas a 35</FONT><FONT FACE=Symbol>&deg;</FONT><FONT FACE="Arial"> C por 48 horas. Pasado este tiempo de incubaci&oacute;n, la secreci&oacute;n vaginal fue negativa, as&iacute; mismo la leche materna del seno derecho y la piel alrededor del pez&oacute;n de este mismo pecho. Con respecto a la leche y a la piel alrededor del pez&oacute;n del seno izquierdo, se logr&oacute; un cultivo positivo por <I>Prototheca wickerhamii.</I></FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">La madre recibi&oacute; fluconazole como tratamiento y se le pidi&oacute; que no alimentara con su leche al ni&ntilde;o. Ambos evolucionaron satisfactoriamente.</FONT></FONT>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>Comentario</FONT></FONT></B>     <BR><FONT SIZE=-1>&nbsp;</FONT>     <BR><FONT SIZE=-1><FONT FACE="Arial">El presente reporte constituye el primero realizado en Costa Rica, en donde se relaciona a la <I>Prototheca wickerhamii</I> con un aislamiento en humanos.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">El caso es una meningitis en un reci&eacute;n nacido cuya madre parece ser el origen de la infecci&oacute;n. Sin embargo la forma como ella adquiri&oacute; el microorganismo, no fue dilucidada. Este caso tiene importancia cl&iacute;nica y epidemiol&oacute;gica, puesto que se logr&oacute; demostrar la presencia del agente en Costa Rica y que se tuvo la suficiente capacidad t&eacute;cnica para aislarlo e identificarlo; adem&aacute;s, se acumul&oacute; experiencia para casos futuros.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial">Tambi&eacute;n es importante mencionar la utilidad que tendr&iacute;a el cultivo de la leche materna en casos de infecciones del per&iacute;odo neonatal.</FONT></FONT>     ]]></body>
