Mar&iacute;a Adilia S&aacute;nchez.<A HREF="#*">*</A></B></FONT></FONT></CENTER>      <CENTER><FONT SIZE=-1>&nbsp;</FONT></CENTER> <FONT SIZE=-1>&nbsp;</FONT>      <P>&nbsp;<B><FONT FACE="Arial"><FONT SIZE=-1>Introducci&oacute;n</FONT></FONT></B>      <P><FONT FACE="Arial"><FONT SIZE=-1>El estudio citogen&eacute;tico es importante para investigar patolog&iacute;as gen&eacute;ticas de &iacute;ndole sexual y en &eacute;l se pueden establecer y definir el diagn&oacute;stico y pron&oacute;stico de un paciente.</FONT></FONT>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>Discusi&oacute;n</FONT></FONT></B>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial"><FONT SIZE=-1>Los cromosomas sexuales se pueden alterar en su n&uacute;mero o en su morfolog&iacute;a: cada cromosoma presenta un brazo corto (p) y un brazo largo (q). Entre las alteraciones morfol&oacute;gicas m&aacute;s importantes est&aacute;n las deleciones, producidas por p&eacute;rdida del material gen&eacute;tico. En el caso del cromosoma X puede ocurrir deleci&oacute;n en el brazo corto (del Xp) o del brazo largo (Xq). Otras alteraciones morfol&oacute;gicas son las translocaciones: consisten en el intercambio del material gen&eacute;tico entre dos cromosomas no hom&oacute;logos y la formaci&oacute;n de isocromosomas que se dan al perderse el brazo largo y tener en el mismo cromosoma dos brazos cortos (iXp) o p&eacute;rdida del brazo corto y ganancia de otro brazo largo (iXq).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Estas anormalidades estructurales o morfol&oacute;gicas del cromosoma X est&aacute;n relacionadas en algunos casos con disgenesia gonadal. Pacientes con un isocromosoma de brazos largos en su mayor&iacute;a presentan caracter&iacute;sticas del S&iacute;ndrome de Turner. Pacientes con deleci&oacute;n del brazo corto pueden presentar disgenesia gonadal aunque algunos pacientes con peque&ntilde;as deleciones terminales presenta solamente baja talla.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Pacientes con deleciones del brazo largo u ocasionalmente pacientes con isocromosoma de brazos cortos del X, muestran disgenesia gonadal pero no baja talla u otras caracter&iacute;sticas som&aacute;ticas del S&iacute;ndrome de Turner.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Existe tambi&eacute;n un grupo de pacientes que presentan disgenecia gonadal con translocaciones entre un autosoma y el brazo largo del cromosoma X (<A HREF="#9">9</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>La regi&oacute;n critica del cromosoma X que se relaciona con disgenesia gonadal involucra del segmentos en el brazo largo entre las bandas q13 a q22 y q22 a q26; separadas por una peque&ntilde;a regi&oacute;n en Xq22 donde, si la quebradura ocurre dentro de esta regi&oacute;n, no se produce la patolog&iacute;a (<A HREF="#12">12</A>). Los s&iacute;ntomas cl&iacute;nicos que acompa&ntilde;an las peque&ntilde;as deleciones distales en Xq (Xq26---qter) pueden provocar desde amenorreas secundarias, menopausia prematura hasta amenorrea primaria (<A HREF="#14">14</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Como resultado de la selecci&oacute;n en la inactivaci&oacute;n del cromosoma X en mujeres, el cromosoma X con alteraciones visibles usualmente es el que se inactiva en todas las c&eacute;lulas som&aacute;ticas. (<A HREF="#7">7</A>). Se han reportado casos de mujeres fenot&iacute;picamente normales con deleciones distales del brazo largo del cromosoma X a nivel Xq22 y desarrollo de pubertad expont&aacute;neo, incluyendo menarca. Esto sugiere que la parte distal del brazo largo del cromosoma X no es crucial para el desarrollo ov&aacute;rico (<A HREF="#16">16</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>A nivel molecular se ha logrado detectar translocaci&oacute;n del gen SRY del cromosoma sexual Y al cromosoma sexual X por lo que se recomienda el uso de fluorescencia de hibridizaci&oacute;n <I>in situ</I> (FISH) as&iacute; como la reacci&oacute;n en cadena de la polimerasa (PCR) en el estudio de los cromosomas sexuales (<A HREF="#10">10</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Una revisi&oacute;n de la literatura demuestra que:</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>* La mayor&iacute;a de las mujeres con inversi&oacute;n del cromosoma X (inv X) son fenot&iacute;picamente normales y f&eacute;rtiles.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>* Pacientes con duplicaci&oacute;n del brazo corto y deleci&oacute;n del brazo largo est&aacute;n asociadas a falla ov&aacute;rica y talla normal o alta. En el caso de mujeres con falla ov&aacute;rica prematura la inducci&oacute;n a la ovulaci&oacute;n es posible (<A HREF="#6">6</A>).</FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial"><FONT SIZE=-1>* La falta del brazo corto del cromosoma X especialmente de las bandas Xcen a Xp11 causan en cl&aacute;sico S&iacute;ndrome de Turner; por otro lado, s&iacute;ntomas turnerarianos individuales incluyendo disgenesia gonadal est&aacute;n involucrados con deleciones en el brazo corto y largo del cromosoma X representando un 65% y un 93% respectivamente; mientras que todos los isodic&eacute;ntricos, tanto en el brazo largo como en el corto (idic Xp, idic Xq), causan amenorrea primaria o secundaria (<A HREF="#13">13</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Se ha sugerido que el fenotipo puede estar estrechamente ligado tanto al patr&oacute;n de replicaci&oacute;n del cromosoma X como a su comportamiento en la inactivaci&oacute;n, el cual puede ser usado para consejo gen&eacute;tico (<A HREF="#4">4</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Deleciones en el cromosoma X resultan generalmente en la presencia de una o muchas caracter&iacute;sticas del S&iacute;ndrome de Turner pero que cada una de estas caracter&iacute;sticas no puede ser atribuida a una deleci&oacute;n espec&iacute;fica. La inactivaci&oacute;n del cromosoma X es un fen&oacute;meno complejo ya que no se da una inactivaci&oacute;n completa, sino que se dan centros de inactivaci&oacute;n y centros de expresi&oacute;n de ciertos genes sobre el cromosoma inactivo. Eso explicar&iacute;a la ausencia o manifestaci&oacute;n de s&iacute;ntomas cl&iacute;nicos en una paciente portadora de un cromosoma X delecionado (<A HREF="#11">11</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Se ha descrito una familia en la cual cuatro mujeres tuvieron irregularidades menstruales y una deleci&oacute;n parcial del brazo largo del cromosoma X; tres de ellas tuvieron falla ov&aacute;rica de los 24 a los 37 a&ntilde;os. Al ser estas pacientes analizadas mediante cariotipos se sugiri&oacute; que presentaban delecionada una parte terminal del Xq. Sin embargo, al realizar estudio por hibridizaci&oacute;n del ADN se demostr&oacute; que en realidad la secci&oacute;n delecionada era una parte intersticial del Xq. Estos hallazgos manifiestan el papel del Xq en la funci&oacute;n ov&aacute;rica y que la agudeza de la citogen&eacute;tica debe ir acompa&ntilde;ada del an&aacute;lisis molecular del ADN (<A HREF="#5">5</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Tambi&eacute;n se han reportado casos de menopausia prematura debido a una deleci&oacute;n heredada en Xq por lo que la presencia de casos familiares sugiere que debe realizarse una investigaci&oacute;n citogen&eacute;tica en aquellos casos de mujeres j&oacute;venes que sufren oligoamenorrea, especialmente cuando sus madres han experimentado menopausia prematura (<A HREF="#15">15</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Se han reportado casos de pacientes f&eacute;rtiles con deleciones en Xq 25, lo que sugiere una posible exepci&oacute;n para la hip&oacute;tesis de la regi&oacute;n cr&iacute;tica para el desarrollo ov&aacute;rico (<A HREF="#8">8</A>).</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>Otra condici&oacute;n importante de anotar es la relacionada a casos de pacientes hermafroditas verdaderos (pacientes que presentan tejido testicular y ov&aacute;rico) tanto las estructuras Muellerianas como las de Wolff est&aacute;n usualmente presentes y sus genitales externos son a menudo ambiguos. Se han propuesto 2 mecanismos alternos para explicar el desarrollo testicular de estos individuos:</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>a) Translocaci&oacute;n del gen que codifica para el factor determinante testicular (TDF) del cromosoma Y al cromosoma X o a un autosoma.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>b) Mutaci&oacute;n en ausencia de TDF que permite el desarrollo testicular.</FONT></FONT>      <P><FONT FACE="Arial"><FONT SIZE=-1>En estos casos el fenotipo se debe a una condici&oacute;n gen&eacute;ticamente heterog&eacute;nea (<A HREF="#1">1</A>) por lo que se debe realizar adem&aacute;s del estudio citogen&eacute;tico, al an&aacute;lisis molecular.</FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial"><FONT SIZE=-1>Se ha descrito un caso de una paciente con disgenesia gonadal parcial que inclu&iacute;a al lado derecho test&iacute;culos rudimentarios y al lado izquierdo &uacute;tero y trompas de falopio poco desarrollados, as&iacute; como estigmatas del S&iacute;ndrome de Turner. Se le logr&oacute; determinar la presencia de un marcador cromos&oacute;mico cuyo an&aacute;lisis molecular evidenci&oacute; que se trataba de una secci&oacute;n del brazo corto del cromosoma Yque inclu&iacute;a al SRY, ZFY, pY-190 con p&eacute;rdida de m&aacute;s del 75%del brazo largo. La disgenesia gonadal, as&iacute; como el fenotipo Turneriano en esta paciente es sugerido por la presencia de genes "anti-Turner" cercanos al TDF. Defectos cromos&oacute;micos en las secuencias flanqueantes al TDF podr&aacute;n provocar la reducida funcionalidad tanto del TDF como de estos genes "anti-Turner" (<A HREF="#3">3</A>).</FONT></FONT>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>Conclusiones</FONT></FONT></B>      <P><FONT FACE="Arial"><FONT SIZE=-1>Los cromosomas sexuales X e Y son los cromosomas objetivo de estudio cuando se presentan problemas en el desarrollo sexual del individuo; sin embargo, debido a que muchas de estas patolog&iacute;as pueden ser producidas por alteraciones en los autosomas tambi&eacute;n debe prestarse atenci&oacute;n a estos &uacute;ltimos. Las alteraciones en el cromosoma X pueden ser en n&uacute;mero como cuando ocurre la p&eacute;rdida de un cromosoma X produci&eacute;ndose la monosom&iacute;a del X o bien tambi&eacute;n puede presentar alteraciones estructurales, las cuales son m&aacute;s complejas y su estudio requiere de mayor experiencia citogen&eacute;tica. Entre estas aberraciones podemos encontrar formaci&oacute;n del isocromosoma de brazos largos del X, cromosoma en anillo o deleciones. Estas &uacute;ltimas son las que llaman nuestra atenci&oacute;n en la presente revisi&oacute;n bibliogr&aacute;fica ya que nos permite relacionar y concluir el diagn&oacute;stico y el pron&oacute;stico del caso. La biolog&iacute;a molecular es de gran ayuda en la detecci&oacute;n de genes a lo largo del cromosoma X y su funci&oacute;n. Es muy importante detectar no s&oacute;lo si existe una alteraci&oacute;n estructural sino tambi&eacute;n a qu&eacute; nivel cromos&oacute;mico ocurri&oacute; y definir el &aacute;rea delecionada. Ya que es interesante anotar como existen pacientes con alteraciones estructurales del cromosoma X y que son f&eacute;rtiles presentando solamente irregularidades menstruales o menopausia prematura y lo que es m&aacute;s interesante es que se han reportado casos de herencia familiar donde el consejo gen&eacute;tico tiene importante participaci&oacute;n. Los an&aacute;lisis de diagn&oacute;stico por ADN deben ser aplicados en casos de marcadores cromos&oacute;micos y aberraciones de los cromosomas X e Y (<A HREF="#2">2</A>).</FONT></FONT>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial"><FONT SIZE=-1>Bibliograf&iacute;a</FONT></FONT></B>     <BR>&nbsp;     <!-- ref --><BR><A NAME="1"></A><FONT FACE="Arial"><FONT SIZE=-1>1.
