Se introdujo una técnica recientemente desarrollada para estudios micológicos y concentración mínima inhibitoria, sólo en organismos levaduriformes, ya que subsisten problemas con la preparación de inóculos con micelios. Se estudiaron 100 cepas de C. albicans y se encontró que no había resistentes a Anfotericina B, pero si un 25,5% con sensibilidad intermedia. Se llama la atención sobre ese inicio de desarrollo de resistencia y la necesidad de un uso más racional de la Anfotericina B.]]></p></abstract>

Isabel Duarte<A NAME="*a"></A><A HREF="#*">*</A>&nbsp;&nbsp; y&nbsp;&nbsp; Dr. Marco Luis Herrera<A HREF="#*">*</A></FONT></FONT></B></CENTER>      <CENTER>&nbsp;</CENTER>      <CENTER><FONT FACE="Arial,Helvetica">&nbsp;</FONT></CENTER> <B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Introducci&oacute;n</FONT></FONT></B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En los &uacute;ltimos a&ntilde;os, hemos observado un aumento significativo en el aislamiento de <I>Candida albicans</I> y otras especies de este g&eacute;nero, especialmente en pacientes inmunosupresos (<A HREF="#8">8</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Este aumento puede ser atribuido al avance en la sobrevida de los reci&eacute;n nacidos de muy bajo peso; al logrado en el campo de los transplante, ya sean de &oacute;rganos s&oacute;lidos o de m&eacute;dula &oacute;sea, y al aumento indudable en la sobrevida de pacientes con c&aacute;ncer. Otro importante factor, es el aumento indiscriminado en el uso de antibi&oacute;ticos antibacterianos de amplio espectro. Adem&aacute;s, no podemos dejar de lado el aumento continuo de personas con sida (<A HREF="#4">4</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La mejor&iacute;a de los protocolos para el manejo del sida, c&aacute;ncer y transplantes de &oacute;rganos, ha aumentado la supervivencia de estas personas, pero las ha hecho m&aacute;s susceptibles al ataque de organismos oportunistas como la <I>Candida albicans </I>(<A HREF="#4">4</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El desarrollo de las drogas antimic&oacute;ticas ha sido lento y son relativamente pocas las alternativas con las que se cuenta para el tratamiento de infecciones mic&oacute;ticas invasivas.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Pr&aacute;cticamente son seis los agentes antif&uacute;ngicos: los Polienos como Anfotericina B, los Imidazoles como Miconazole y Ketoconazole, los Triazoles como Fluconazole e Itraconazole y los inhibidores de la s&iacute;ntesis de pirimidinas como la Flucitocina (5-FC) (<A HREF="#4">4</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>De todos estos antimic&oacute;ticos, Anfotericina B, es el de mayor uso y act&uacute;a inhibiendo el ergosterol de la membrana celular del hongo, causando lisis de sus c&eacute;lulas. Es necesario recordar, que el hongo como eucariota que es, presenta una membrana celular id&eacute;ntica a las de las c&eacute;lulas humanas, raz&oacute;n por la cual esta dorga es un antimic&oacute;tico de alta toxicidad.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Dentro de sus efectos colaterales, podemos observar la fiebre, n&aacute;useas, v&oacute;mitos, anorexia, escalofr&iacute;os, trombocitopenia, anemina, tromboflevitis y el m&aacute;s serio: la nefropat&iacute;a (<A HREF="#2">2</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>A pesar de este hecho, Anfotericina B sigue siendo el tratamiento de elecci&oacute;n para la <I>Candida albicans </I>y otros agentes mic&oacute;ticos especialmente los organismos dim&oacute;rficos (<A HREF="#7">7</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los azoles como Miconazole, Ketoconazole, Itraconazole y el Fluconazole son alternativas viables, pero su actividad no ha logrado sustituir claramente a la Anfotericina B (<A HREF="#10">10</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Esto hace necesario realizar la determinaci&oacute;n "in vitro" de la Concentraci&oacute;n M&iacute;nima Inhibitoria (CMI), de la Anfotericina B contra la <I>Candida albicans</I> y de esta forma tratar de disminuir al m&aacute;ximo los efectos adversos de esta droga; adem&aacute;s ya hay reportes de cepas de <I>Candida albicans</I> resistentes a la Anfotericina B (<A HREF="#1">1</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El campo micol&oacute;gico de la sensibilidad "in vitro", ha sido un punto de mucha complejidad, con una serie enorme de problemas a resolver.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Para realizar estas pruebas de sensibilidad, se han utilizado tres m&eacute;todos diferentes, macro y micro diluci&oacute;n en agar y tiras de E test (<A HREF="#4">4</A>).</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     ]]></body>
<body><![CDATA[<BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Material y M&eacute;todos</FONT></FONT></B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En este estudio, se utilizaron 100 cepas de <I>Candida albicans</I>, aisladas en 1994 en el Laboratorio Cl&iacute;nico del Hospital de Ni&ntilde;os e identificadas con el Sistema API 20C (Biomerieux) o utilizando tarjetas Yeast para el Sistema Automatizado Vitek (Biomerieux).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>De estas 100 cepas, 20 fueron aisladas a partir de adultos, todos ellos empleados del Hospital de Ni&ntilde;os y se distribuyeron de la siguiente manera: 5 de lesiones en piel (manos), 5 de infecciones ungueales y 10 de secreci&oacute;n vaginal.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Las 80 cepas restantes, fueron aisladas de ni&ntilde;os internados en los diferentes servicios del Hospital Nacional de Ni&ntilde;os y se distribuyeron de la siguiente manera: fungemia 14 cepas, secreci&oacute;n bronquial 11, mucosa oral 10, secreci&oacute;n vaginal 10, orina 10, secreci&oacute;n traqueal 9, abscesos peritoneales 8, otitis media cr&oacute;nica 5, ungueales 2 y una cepa de l&iacute;quido pleural. En todos &eacute;stos, se document&oacute; la significancia cl&iacute;nica del agente aislado, en base a criterios cl&iacute;nicos como la fiebre y de laboratorio como fue la alteraci&oacute;n del hemograma.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La CMI, fue definida como la m&iacute;nima concentraci&oacute;n de Anfotericina B que inhibe el crecimiento visible de un in&oacute;culo estandarizado de <I>Candida albicans</I>.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El medio de cultivo usado fue RMPI 1640 m&aacute;s alfa glutamina y un tamp&oacute;n de fosfatos (Sigma Chemical Co. r6405). Se prepararon al&iacute;cuotas con 10,4 g de medio disolvi&eacute;ndolo en un litro de agua bidestilada y amortiguado con N-2 hidroxithylpiperazine N-2 ethanesulfonic acid, (HEPES, Sigma Chemical Co.) a pH 7,0 (<A HREF="#9">9</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Este medio de cultivo se esteriliz&oacute; por filtraci&oacute;n y se almacen&oacute; a -70&ordm;C en botellas de color &aacute;mbar, previa inoculaci&oacute;n de una al&iacute;cuota de 0,1 ml de tioglicolato, el cual se incub&oacute; por 7 d&iacute;as para asegurar esterilidad.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Para la preparaci&oacute;n del in&oacute;culo se us&oacute; el m&eacute;todo espectrofotom&eacute;trico (<A HREF="#11">11</A>), para lo cual las cepas de <I>Candida</I> fueron cultivadas sobre agar sangre, hecho a base de eritrocitos humanos al 5% e incubadas a 35 grados cent&iacute;grados por 24 o 48 horas.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El in&oacute;culo se estandariz&oacute; entre 1 x 10<SUP>5</SUP> y 5 x 10<SUP>5</SUP> ufc/ml utilizando un nefel&oacute;metro Vitek y se hizo en RPMI; a partir de all&iacute; se realiz&oacute; una diluci&oacute;n de 1:2.000, para un in&oacute;culo final de 0,5 x 10<SUP>3</SUP> a 2,5 x 10<SUP>3</SUP> ufc/ml (3, 4, 6, 11).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Las placas para la microtitulaci&oacute;n fueron de 96 pozos con fondo en U (Dynatech Laboratories Inc. Alexandria VA) y en todos los pozos se colocaron 50 ul de medios de cultivo (RPMI).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La soluci&oacute;n madre de Anfotericina B (Squibb and Sons, Princeton, N.J.) se prepar&oacute; a 2.560 ug/ml.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Ya en la placa, en el pozo 1 se colocaron 50 ul de la soluci&oacute;n madre dilu&iacute;da de 1:10 y de all&iacute; en adelante se realizaron diluciones dobles, pasando 50 ul de un pozo a otro, hasta el pozo 10 dejando el 11 y 12 como control positivo y negativo respectivamente.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Una vez realizadas las diluciones, y respetando el control negativo, se colocaron 50 ul de in&oacute;culo de la levadura.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>De esta manera, el pozo 1 ten&iacute;a una concentraci&oacute;n de 128 ug/ml y el pozo 10 de 0,25 ug/ml.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La placa se sell&oacute; y se incub&oacute; a 35 grados cent&iacute;grados por 48 a 72 horas sin agitaci&oacute;n, siendo el control positivo el que nos dio el tiempo de lectura.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Pasado este lapso, la placa se ley&oacute; por turbiedad, siempre referido a los controles, tanto positivo como negativo, utilizando un espejo lector (Lector Cooke Engineering Co., Alexandria VA).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Una CMI para Anfotericina B, se considera sensible si es menor de 2 ug/ml; intermedia, si se encuentra entre 2 y 4 ug/ml; y resistente si la CMI es mayor de 4 ug/ml (<A HREF="#4">4</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Como control positivo de crecimiento y reproducibilidad, se utiliz&oacute; la cepa control de la <I>Candida albicans </I>ATCC 90028, que representa una sensibilidad a la Anfotericina B entre 0,25 y 1,0 ug/ml (<A HREF="#2">2</A>).</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     ]]></body>
<body><![CDATA[<BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Resultados</FONT></FONT></B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En este estudio, tanto las cepas aisladas de adultos como las aisladas de ni&ntilde;os, mostraron resultados muy semejantes, por lo que no haremos referencia de su diferenciaci&oacute;n.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>De las 100 cepas estudiadas, un 74,8% mostraron sensibilidad a la Anfotericina B, con un CMI menor o igual a 1 ug/ml.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El 25,5% mostraron una sensibilidad intermedia con una CMI entre 2 y 4 ug/ml.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>No se encontraron cepas en niveles de resistencia (CMI>4,0 ug/ml).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En todas las oportunidades en las que se mont&oacute; la prueba, la cepa de <I>Candida albicans</I>, ATCC 90028, mostr&oacute; los niveles esperados y la mayor parte de las veces la CMI fue de 0,5 ug/ml.</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1><B>&nbsp;&nbsp;</B></FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Conclusiones</FONT></FONT></B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La <I>Candida albicans</I> constituye un problema importante de salud a nivel mundial (<A HREF="#4">4</A>). Son cada vez m&aacute;s frecuentes sus aislamientos y su capacidad invasiva es ampliamente reconocida (<A HREF="#4">4</A>,<A HREF="#12">12</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>No cabe duda, de que los pacientes inmunosupresos son las principales v&iacute;ctimas de este agente, el cual es un claro ejemplo de un organismo oportunista (<A HREF="#12">12</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Para la mayor&iacute;a de los organismos mic&oacute;ticos, Anfotericina B, es el antimic&oacute;tico de elecci&oacute;n, a pesar de la gran cantidad de efectos colaterales de esta droga. Nuevas alternativas como los azoles y especialmente Fluconazole, no han demostrado una mejor actividad antimic&oacute;tica que Anfotericina B. Sin embargo, contra las levaduras, la actividad del Fluconazole es bastante buena, sin lograr desplazar a la Anfotericina B.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El riesgo de tratamientos con Anfotericina B es alto; sin embargo, su utilidad es evidente.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Estas son las razones principales para medir la actividad "in vitro" de esta droga y establecer cu&aacute;l es la m&iacute;nima concentraci&oacute;n a nivel de tisular, necesaria para inhibir el crecimiento del agente etiol&oacute;gico.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Las pruebas de sensibilidad para antimic&oacute;ticos, presentan problemas como son el tama&ntilde;o del in&oacute;culo, la preparaci&oacute;n de &eacute;ste, la formulaci&oacute;pn del medio de cultivo empleado y su pH, la duraci&oacute;n y la temperatura de incubaci&oacute;ny los criterios para la determinaci&oacute;n de los puntos finales de la CMI (<A HREF="#3">3</A>,<A HREF="#4">4</A>,<A HREF="#6">6</A>,<A HREF="#11">11</A>). A esto hay que agregar el hecho de que los organismos mic&oacute;ticos tienen una morfolog&iacute;a o forma de desarrollo muy diferente al resto de los microorganismos (<A HREF="#4">4</A>). Tampoco podemos dejar de lado las propiedades del antimic&oacute;tico como son la solubilidad, estabilidad qu&iacute;mica y mecanismos de acci&oacute;n (<A HREF="#4">4</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En este estudio, tratamos de introducir una t&eacute;cnica recientemente desarrollada para estudios micol&oacute;gicos y CMI, pero que est&aacute; disponible s&oacute;lo para organismos levaduriformes, ya que aun hay problemas a resolver a la hora de preparar el in&oacute;culo en organismos miceliales (<A HREF="#4">4</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La metodolog&iacute;a seguida en este estudio es la recomendada a nivel mundial, tanto para el medio de cultivo, como para la preparaci&oacute;n del in&oacute;culo, el tiempo de incubaci&oacute;n y la determinaci&oacute;n de los puntos finales para la CMI (<A HREF="#3">3</A>,<A HREF="#4">4</A>,<A HREF="#6">6</A>,<A HREF="#11">11</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>S&oacute;lo fue necesario hacer un ajuste en el amortiguador usado, ya que el recomendado a nivel mundial (MOPS) (<A HREF="#4">4</A>), no se consigue en nuestro pa&iacute;s. Por lo que se cambi&oacute; este amortiguador por el HEPES, siempre respetando el pH de 7,0.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La t&eacute;cnica de microtitulaci&oacute;n para la determinaci&oacute;n de la CMI para Anfotericina B contra <I>Candida albicans</I>, es una t&eacute;cnica complicada y que consume mucho tiempo de trabajo, por lo que una nueva t&eacute;cnica desarrollada a base de las tiras de E test, y siempre para organismos levaduriformes pone a nuestra disposici&oacute;n, una forma m&aacute;s sencilla de realizar la CMI; sin embargo, esta tiras son de alto precio (<A HREF="#5">5</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Una de las principales intenciones de este estudio era la de comprobar, si en el Hospital Nacional de Ni&ntilde;os, es posible encontrar organismos del g&eacute;nero <I>Candida</I> resistente a la Anfotericina B. De las cien cepas, un 25,2% presentaron sensibilidad intermedia, y las 74,8% restante fueron sensibles.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
