Material y Métodos: Mediante las técnicas de ELISA y RIA se hizo determinación de la presencia de tres marcadores del VHB en el suero de 336 pacientes fallecidos. Resultados: Se encontró HBsAg en el 2,4% de los casos; anticuerpos anti-HBs en el 29,8%; y anti-HBc en el 31,8%. Conclusiones: La frecuencia de los tres marcadores mencionados fue más alta en esta casuística, que en otras de personas vivas del país descritas por otros autores. Los autores no pudieron ubicar investigaciones similares a ésta, en personas hospitalizadas que mueren en nosocomios. Este estudio sugiere que la infección con VHB es más frecuente en la población hospitalizada que muere en el hospital, que en la población general.]]></p></abstract>

Material and Methods: Using ELISA and RIA methods, three serological markers of HBV were investigated in the serum of the corpses. Results: HBsAg was found in 2.4% of the deceased, anti-HBs in 29.8%; and anti-HBc in 31.8%. Conclusions: The frequency of the markers studied was relatively high in this sample, in comparison with rates in blood donors and other population groups in the country. M&aacute;s de 2000 millones de personas saben que tienen o han tenido una infecci&oacute;n por el VHB y 350 millones de personas son portadoras cr&oacute;nicas del virus (<A HREF="#4">4</A>) en tanto aparecen 2,5 millones de nuevas infecciones de ese virus por a&ntilde;o (<A HREF="#5">5</A>). El VHB es una causa importante de hepatitis aguda y cr&oacute;nica y de cirrosis hep&aacute;tica, y puede ser la causa de hasta el 80% del carcinoma hepatocelular, que es el c&aacute;ncer m&aacute;s importante en varias regiones del mundo (<A HREF="#10">10</A>, <A HREF="#11">11</A>). El VHB ocupa el segundo lugar despu&eacute;s del tabaco; entre los carcin&oacute;genos identificados en el humano (<A HREF="#6">6</A>, <A HREF="#7">7</A>). S&oacute;lo en los Estados Unidos se calcula que hay cerca de 1.25 millones de portadores cr&oacute;nicos del VHB (<A HREF="#6">6</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">La infecci&oacute;n con VHB constituye la causa principal de enfermedad hep&aacute;tica, generando una gran carga econ&oacute;mica en los servicios de salud de pa&iacute;ses industrializados y en desarrollo. En &eacute;stos, la prevalencia global de portadores oscila entre 5 y 20% (<A HREF="#8">8</A>, <A HREF="#9">9</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">Aunque el VHB fue reconocido inicialmente como un agente transmitido por transfusiones o punzadas con agujas recicladas y otros materiales contaminados con sangre, desde la d&eacute;cada de 1970 se reconoci&oacute; que la mayor&iacute;a de las infecciones se adquieren por contacto sexual (<A HREF="#12">12</A>, <A HREF="#13">13</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">La infecci&oacute;n con el VHB (familia Hepadnaviridae), se refleja en sus marcadores serol&oacute;gicos: ant&iacute;geno de superficie (HBsAg), anticuerpos anti-ant&iacute;geno de superficie (anti-HBs), ant&iacute;geno central (HBcAg), anticuerpos anti-ant&iacute;geno central (anti-HBc), ant&iacute;geno e (HBeAg), anticuerpos anti-ant&iacute;geno e (anti-HBe) y ADN viral (<A HREF="#14">14</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">La presencia del HBeAg denota s&iacute;ntesis viral activa y por lo tanto alta infectividad. Los anticuerpos anti-e, que reflejan una baja tasa de infectividad, que aparecen en la fase integrada de la infecci&oacute;n, cuando el ADN del virus forma parte del material gen&eacute;tico de las c&eacute;lulas hu&eacute;sped (<A HREF="#15">15</A>, <A HREF="#16">16</A>). Los anticuerpos anti-HBc totales se presentan en casi todos los casos de hepatitis cr&oacute;nica. Los anticuerpos anti-HBc de forma 7-8S, tipo IgM predominan en la infecci&oacute;n cr&oacute;nica, mientras que los de forma 19S predominan en la hepatitis aguda (<A HREF="#8">8</A>, <A HREF="#24">24</A>, <A HREF="#25">25</A>). La persistencia del HBsAg en sangre, durante seis meses o m&aacute;s, despu&eacute;s de su detecci&oacute;n inicial, permite el diagn&oacute;stico de laboratorio definitivo del proceso cr&oacute;nico. La presencia del marcador, sin alteraci&oacute;n de transaminasas ni otros signos sugestivos de inflamaci&oacute;n hep&aacute;tica, caracteriza al "estado de portador cr&oacute;nico sano" (<A HREF="#15">15</A>, <A HREF="#17">17</A>). Los t&iacute;tulos bajos de IgG anti-HBc y de anti-HBs indican una infecci&oacute;n vieja de VHB. Los t&iacute;tulos altos de IgG anti-HBc sin anti-HBs denotan una infecci&oacute;n viral persistente (<A HREF="#8">8</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">El presente estudio tuvo como objetivo conocer la frecuencia y varias caracter&iacute;sticas de la infecci&oacute;n con el VHB en una poblaci&oacute;n de adultos reci&eacute;n fallecidos en un hospital Clase A de San Jos&eacute;. No conocemos estudios similares en Costa Rica; por lo que se deseaba conocer si la infecci&oacute;n con este virus en esta poblaci&oacute;n ten&iacute;a caracter&iacute;sticas similares o no con respecto a otros grupos de poblaci&oacute;n estudiados en el pa&iacute;s.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><B><FONT FACE="Arial,Helvetica">&nbsp;</FONT></B></FONT>     ]]></body>
<body><![CDATA[<BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Material y m&eacute;todos</FONT></FONT></B>     <BR><FONT SIZE=-1><FONT FACE="Arial,Helvetica">&nbsp;</FONT></FONT>     <BR><FONT SIZE=-1><FONT FACE="Arial,Helvetica">Se estudiaron 1083 &oacute;bitos acaecidos en el lapso de dieciocho meses en el Hospital Dr. R.A. Calder&oacute;n Guardia, que representan casi todos los adultos fallecidos de junio de 1993 a diciembre de 1994. De esta serie se seleccion&oacute; al azar 336 casos que son el objeto de este informe. La incorporaci&oacute;n de casos se hizo en la morgue del Servicio de Patolog&iacute;a. Para cada caso se consign&oacute;, en un formulario precodificado el n&uacute;mero del expediente, lugar de residencia de la persona, sexo, edad, estado civil, ocupaci&oacute;n, y causas de muerte. No se registr&oacute; el nombre de la persona. Adem&aacute;s, se anot&oacute; si hab&iacute;a antecedentes de hepatitis, manifestaciones cl&iacute;nicas de esa infecci&oacute;n, y los resultados de pruebas de funci&oacute;n hep&aacute;tica durante la hospitalizaci&oacute;n. Adem&aacute;s, se consign&oacute; si el individuo hab&iacute;a recibido transfusiones de sangre o de hemoderivados.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">Los &oacute;bitos fueron transportados poco tiempo despu&eacute;s del fallecimiento, al Servicio de Patolog&iacute;a del hospital. Ah&iacute;, se recolect&oacute; de cada cad&aacute;ver una muestra de sangre de la vena yugular externa, previa limpieza de la piel con agua, jab&oacute;n l&iacute;quido y alcohol de 70%. La sangre fue centrifugada a 2.200xg y a 4&ordm;C durante diez minutos. Los sueros fueron alicuotados en viales de una dracma, y almacenados a -70&ordm;C. Para medir los marcadores serol&oacute;gicos del VHB se utiliz&oacute; la t&eacute;cnica del ensayo inmunoenzim&aacute;tico (ELISA) (<A HREF="#14">14</A>, <A HREF="#15">15</A>, <A HREF="#17">17</A>) empleando estuches Abbott para HBsAg (AUSZYME), anti-HBs (AUSAB) y anti-HBc (CORZYME). La lectura de las reacciones se hizo en un espectrofot&oacute;metro Abbott (Quantum II<SUP>TM</SUP>). Los sueros reactivos al ant&iacute;geno de superficie en ELISA fueron analizados por radioinmunoensayo (RIA)(<A HREF="#14">14</A>, <A HREF="#23">23</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">Los datos fueron analizados utilizando el programa "Statistical Package for the Social Sciences". Se emple&oacute; la prueba exacta de Fisher para establecer significaci&oacute;n estad&iacute;stica.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">&nbsp;</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Resultados</FONT></FONT></B><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">De los 336 decesos, 186 (55,3%) eran hombres y 150 (44,6%) mujeres (p>0.05). Los sueros de 12 &oacute;bitos (3,6%) fueron reactivos al HBsAg por ELISA, 100 (29,8%) mostraron anticuerpos anti-HBs y 107 (31,8%) anticuerpos anti-HBc (<A HREF="#CUADRO1">Cuadro 1</A>). Cuatro de los 12 casos HBsAg-reactivos por ELISA, fueron negativos por RIA, lo que, de aceptarse este criterio m&aacute;s r&iacute;gido, reduce la frecuencia de HBsAg a 2,4%.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">No se encontr&oacute; ning&uacute;n caso con HBsAg y anti-HBs simult&aacute;neamente; en ausencia de anti-HBc, en dos casos (0,6%) se encontr&oacute; los tres marcadores y en otros dos (0,6%) hab&iacute;a HBsAg y anti-HBc. Solo en cuatro casos (1,2%) se present&oacute; HBsAg como &uacute;nico marcador; veintis&eacute;is (7,7%) ten&iacute;an solo anti-HBs; treinta y dos (9,5%) solo anti-HBc; setenta y uno (21,1%) ten&iacute;an anti-HBs y anti-HBc; y en 137 (40,8%) se hall&oacute; uno o varios marcadores. Ciento noventa y nueve (59,2%) sueros de los 336 &oacute;bitos no ten&iacute;an ninguno de los tres marcadores (<A HREF="#CUADRO 2">Cuadro 2</A>).</FONT></FONT>     <BR><FONT SIZE=-1>&nbsp;</FONT>     ]]></body>

Pero en el 82% de los casos con alg&uacute;n marcador del VHB se hab&iacute;a encontrado pruebas de funci&oacute;n hep&aacute;tica alteradas; no obstante, no hubo diferencia con lo observado en la poblaci&oacute;n sin marcadores al virus.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">Pr&aacute;cticamente todos los pacientes fallecidos proven&iacute;an de las &aacute;reas de atracci&oacute;n del Hospital Calder&oacute;n Guardia que son el sector este de San Jos&eacute;, y las provincias de Cartago y Lim&oacute;n, lo que no permiti&oacute; hacer una proyecci&oacute;n comparativa de la frecuencia de marcadores de hepatitis B en el &aacute;mbito nacional.</FONT></FONT>     <BR><FONT SIZE=-1><B><FONT FACE="Arial,Helvetica">&nbsp;</FONT></B></FONT><FONT SIZE=-1></FONT>      <P><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Discusi&oacute;n</FONT></FONT></B><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">Se encontr&oacute; una frecuencia de 2,4% de HBsAg en la poblaci&oacute;n de fallecidos, que es 7 a 9 veces mayor que la descrita en donadores de sangre en Costa Rica (<A HREF="#1">1-3</A>). La diferencia es l&oacute;gica en tanto los decesos representan una poblaci&oacute;n de mayor promedio de edad, y mayor riesgo de patolog&iacute;a que la de los donadores, quienes en su gran mayor&iacute;a son m&aacute;s j&oacute;venes y est&aacute;n sanos.</FONT></FONT><FONT SIZE=-1></FONT>      ]]></body>
<body><![CDATA[<P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">Muchas de las personas en el estudio padec&iacute;a de enfermedades cr&oacute;nicas, que generalmente les hab&iacute;a llevado a la muerte. En contraste, los donadores de sangre, aunque no totalmente representativos de la poblaci&oacute;n general, se aproximan m&aacute;s a &eacute;sta.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">Se encontr&oacute; una frecuencia de anti-HBs de 29,8%, un anticuerpo que resulta de la infecci&oacute;n con el VHB en alg&uacute;n momento de la vida, dejando inmunidad permanente al virus (<A HREF="#8">8</A>, <A HREF="#18">18</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">En cuatro casos (1,2%) con HBsAg como &uacute;nico marcador, no se pudo establecer con precisi&oacute;n si hab&iacute;a ocurrido un proceso agudo o cr&oacute;nico de la virosis, ya que no se consignaron datos de la evoluci&oacute;n del posible proceso infeccioso por el VHB. Se sabe que en el 5% de los pacientes con hepatitis resuelta, no cr&oacute;nica, puede aparecer sin anticuerpos anti-HBs (<A HREF="#18">18</A>). Dos sujetos (0,6%) mostraron los tres marcadores del VHB, una situaci&oacute;n que puede ocurrir cuando empieza a desaparecer el HBsAg y hay s&iacute;ntesis de anti-HBs; no obstante, lo usual es que el HBsAg no se detecte cuando aparece el anti-HBs, por lo que el anti-HBc es el &uacute;nico marcador normalmente presente en ese per&iacute;odo (<A HREF="#8">8</A>).</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">En la casu&iacute;stica de &oacute;bitos no se encontr&oacute; ning&uacute;n caso con HBsAg y anti-HBs simult&aacute;neamente, sin la presencia del anti-HBc. En ausencia de anti-HBc, la presencia simult&aacute;nea de estos marcadores puede ocurrir en el 20% y 40% de los portadores cr&oacute;nicos (<A HREF="#19">19</A>), por lo que el anticuerpo anti-HBs no excluye la posibilidad de que el proceso haya persistido.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">El marcador anti-HBc siempre est&aacute; presente cuando aparece el cuadro agudo de hepatitis (<A HREF="#19">19</A>, <A HREF="#20">20</A>). En el presente estudio este marcador fue hallado en 106 &oacute;bitos (31.8%), lo que sugiere que casi la tercera parte de la poblaci&oacute;n estudiada hab&iacute;a estado expuesta y fue infectada por el virus. Sin embargo, los datos no permiten establecer si la infecci&oacute;n era aguda o cr&oacute;nica, ya que no se hizo determinaci&oacute;n de anticuerpos anti-HBc en IgM, en tanto los anticuerpos totales anti-HBc persisten en portadores cr&oacute;nicos(<A HREF="#21">21</A>). Los anti-HBc en IgM o totales son detectables cuando desaparece el HBsAg y aun no ha aparecido el anti-HBs (<A HREF="#21">21</A>). Este marcador se hall&oacute; en ausencia de otros en 32 casos (9%), lo que puede ocurrir en infecciones agudas, que cursan con un per&iacute;odo de ventana muy prolongado. El fen&oacute;meno se presenta cuando no es detectable el HBsAg y todav&iacute;a no aparece el anti-HBs. Tambi&eacute;n puede observarse en portadores cr&oacute;nicos, incluso cirr&oacute;ticos cuando han perdido su HBsAg, y aparecen los anti-HBs, hasta a&ntilde;os despu&eacute;s de la infecci&oacute;n. El anti-HBc tambi&eacute;n puede presentarse solo, cuando hay niveles no detectables de HBsAg en el suero (<A HREF="#22">22</A>). En estos casos se ha encontrado HBsAg, ant&iacute;geno central del VHB (HBcAg) en tejido hep&aacute;tico y ADN viral en el suero (<A HREF="#22">22</A>). En el presente estudio no se consider&oacute; necesario hacer determinaci&oacute;n de anti-HBc en IgM para establecer si la infecci&oacute;n era aguda o cr&oacute;nica, obviamente porque la poblaci&oacute;n era de fallecidos.</FONT></FONT><FONT SIZE=-1></FONT>      <P><FONT SIZE=-1><FONT FACE="Arial,Helvetica">Tomando en cuenta que se sabe que la gran mayor&iacute;a de las infecciones con VHB son asintom&aacute;ticas por un lado, y por otro, que los grupos de poblaci&oacute;n de alto riesgo son tambi&eacute;n conocidos, se recomienda implementar un programa de tamizaje serol&oacute;gico y de vacunaci&oacute;n de las poblaciones con mayor riesgo.
