Las mujeres presentaron niveles significativamente mayores ( p < 0,05) de colesterol total (CT), colesterol LDL (C-LDL) y colesterol HDL (C-HDL) que los hombres. Los valores de CT, C-LDL y C-HDL de los adolescentes alumnos de colegios públicos no fueron estadísticamente diferentes a los evidenciados en los estudiantes de los colegios primarios. Un 52% y un 75% de los hombres que presentaron respectivamente, niveles de CT clasificados como altos o moderadamente altos. Asimismo un 47% de los adolescentes mostraron niveles de C-LDL considerados como altos o moderadamente altos. El porcentaje de hombres que presentaron valores bajos de C-HDL fue significativamente mayor ( p< 0,05) que el de las mujeres. El 56% del total de adolescentes incluidos en el estudio mostraron valores mayores de 4.5 para el Indice de Castelli.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The lipidic profile of 204 non-obese healthy seventeen year-old adolescents, from four private and public high schools in the urban area of San José, Costa Rica was studied. Women presented significantly higher levels of total cholesterol (TC), LDL cholesterol (LDL-C) and HDL cholesterol (HDL-C) (p< 0,05) than men. There werw no statistical differences in TC, LDL-C, HDL-C between studens from private high schools and students from public high schools. 52% of men and 75% of women had total cholesterol levels considered elevated or moderately elevated. Smilary, 47% of male adolescents and 59% of female adolescents presented and LDL-C levels considered elevated or moderately elevated. The percentege of males that had low HDL-C levels was significativily higher that females (p< 0,05).]]></p></abstract>

<body><![CDATA[<BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La enfermedad coronaria representa la primera causa de muerte general en Costa Rica, se evidencia un incremento en la tasa de mortalidad por esa enfermedad desde 1989 (<A HREF="#1">1</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Las manifestaciones cl&iacute;nicas de esta patolog&iacute;a se presentan principalmente entre el quinto y sexto decenio de vida; no obstante, las observaciones cl&iacute;nicas, epidemiol&oacute;gicas y anat&oacute;micas, apoyan la hip&oacute;tesis de que las enfermedades cardiovasculares comienzan a gestarse desde la infancia (<A HREF="#2">2</A>,<A HREF="#3">3</A>). Desde 1950 Enos et. Al., Holman et. Al., Strong Mc Gill y otra serie de investigadores han confirmado la presencia de estr&iacute;as de grasa en la aorta en ni&ntilde;os de tres a&ntilde;os y en las arterias coronarias de ni&ntilde;os de 10 a 14 a&ntilde;os (<A HREF="#4">4</A>,<A HREF="#5">5</A>,<A HREF="#6">6</A>). Sin embrago tambi&eacute;n se informa que un 8% de los ni&ntilde;os de 10 a14 a&ntilde;os muestran lesiones ateroscler&oacute;ticas avanzadas (<A HREF="#2">2</A>), lo cual es alarmante pues normalmente las estr&iacute;as grasas progresan a placas fibrosas y otras lesiones m&aacute;s complejas, durante el segundo y tercer decenio de vida (<A HREF="#3">3</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El colesterol plasm&aacute;tico juega un papel central en el desarrollo de la arterioesclerosis (<A HREF="#7">7</A>,<A HREF="#8">8</A>,<A HREF="#9">9</A>), espec&iacute;ficamente cuando las elevaciones corresponden a incrementos de las lipoprote&iacute;nas de baja densidad (C-LDL). Esta asociaci&oacute;n es apoyada por el estudio de Framinghan (<A HREF="#10">10</A>) y por numerosas investigaciones de intervenci&oacute;n sobre factores de riesgo (<A HREF="#8">8</A>,<A HREF="#9">9</A>,<A HREF="#11">11</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Dado lo anterior, este estudio se propuso analizar el perfil l&iacute;pidico de adolescentes de 17 a&ntilde;os, estudiantes de colegios p&uacute;blicos y privados del &aacute;rea urbana de San Jos&eacute;, Costa Rica.</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><B><SUP><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Materiales y m&eacute;todos</FONT></FONT></SUP></B><B><SUP><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT></SUP></B>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Se estudi&oacute; el perfil lip&iacute;dico de 204 adolescentes con edad promedio de 17 a&ntilde;os, sin obesidad u otra patolog&iacute;a, estudiantes de cuatro colegios p&uacute;blicos del &aacute;rea urbana de San Jos&eacute;, Costa Rica.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los colegios incluidos en el estudio se seleccionaron proporcionalmente de acuerdo con el tama&ntilde;o y los estudiantes se eligieron aleatoriamente. En total se escogieron 102 hombres y 104 mujeres. El 50% de la muestra correspondi&oacute; a estudiantes de colegios p&uacute;blicos y el 50% restante a alumnos de centros educativos privados.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Para evidenciar los niveles s&eacute;ricos de l&iacute;pidos se tom&oacute; una muestra de suero sin anticuagulante, despu&eacute;s de 12 horas de ayuno. La determinaci&oacute;n de colesterol total, colesterol HDL y triglic&eacute;ridos se efectu&oacute; por m&eacute;todos enzim&aacute;ticos colorim&eacute;tricos (<A HREF="#12">12</A>), utilizando un analizador ASCA. El nivel de colesterol LDL fue estimado para todos los sujetos con niveles de triglic&eacute;ridos menores de 400 mg/dl usando la ecuaci&oacute;n de Friedwald (<A HREF="#13">13</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los coeficientes de variaci&oacute;n para la prueba de colesterol total de 3,2% para los niveles altos y de 2,85% para los valores normales. En la determinaci&oacute;n del colesterol HDL el coeficiente de variaci&oacute;n fue del 5% y en la de triglic&eacute;ridos la variaci&oacute;n interensayo fue del 2,5% para los niveles altos y 3,5% para aquellos valores normales.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los datos fueron analizados usando la prueba de chi-cuadrado ( p &lt; 0,05).</FONT></FONT>     <CENTER><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT></CENTER>      <CENTER><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></CENTER> <B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Resultados</FONT></FONT></B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los valores promedio de l&iacute;pidos s&eacute;ricos seg&uacute;n sexo y tipo de colegio se presentan en el <A HREF="#CUADRO1">cuadro 1</A>. Las mujeres presentaron niveles de colesterol total (CT), colesterol LDL (C-LDL), colesterol HDL (C-HDL) iguales a 118 <U>+</U> 29 mg/dl y, 48 <U>+ </U>8 mg/dl y 118 <U>+</U>28 mg/dl respectivamente. Estos valores fueron significativamente mayores ( p&lt; 0,05) que los evidenciados en los hombres. El 34% de los hombres mostraron un Indice de Catelli (IC) mayor a 4,5, situaci&oacute;n similar a la observada en el 22% de las mujeres adolescentes. Al clasificar los valores de l&iacute;pidos, seg&uacute;n los niveles de riesgo establecidos por el panel de expertos en niveles de colesterol en ni&ntilde;os y adolescentes de Programa Nacional de Educaci&oacute;n en Colesterol de los Estados Unidos (<A HREF="#2">2</A>), se evidenci&oacute; que un 52% y un 75% de los hombres y mujeres presentaban respectivamente, niveles de CT clasificados como altos o moderadamente altos (<A HREF="#CUADRO2">cuadro 2</A>). Asimismo un 47% de los adolescentes mostraron niveles de C-LDL considerados como altos o moderadamente altos.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Del total de adolescentes estudiados el 56% mostr&oacute; para el IC, valores mayores de 4,5, considerados de riesgo coronario. El porcentaje de hombres con IC mayores a 4,5 fue ligeramente mayor que el de mujeres.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>A&uacute;n cuando el porcentaje de mujeres con niveles altos o moderadamente altos de CT y C-LDL fue mayor que el de hombres, no se encontraron diferencias significativas. Una situaci&oacute;n diferente se observ&oacute; en los niveles de C-HDL, pues el porcentaje de hombres que presentaron valores bajos de esta lipoprote&iacute;na fue significativamente mayor ( p&lt; 0,05) que el de las mujeres.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los valores de CT, C-LDL y C-HDL de los adolescentes alumnos de colegio privados no fueron estad&iacute;sticamente diferentes a los evidenciados en los estudiantes masculinos de los colegios p&uacute;blicos. Sin embargo el porcentaje de hombres (76%) de colegios privados con valores menores a 4,5 para el Indice de Castelli (IC), fue significativamente mayor ( p&lt; 0,05) que el identificado en colegios p&uacute;blicos (52%). Mientras tanto, el porcentaje de hombres de colegios p&uacute;blicos con IC mayores de 4,5 fue significativamente mayor que en los colegios privados. Los niveles de CT, C-LDL, C-HDL e Indice de Castelli de las mujeres no mostraron diferencias estad&iacute;sticamente significativas al analizarlos seg&uacute;n tipo de centro educativo (<A HREF="#CUADRO3">Cuadro 3</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El porcentaje de adolescentes masculinos estudiantes de colegios p&uacute;blicos con niveles de CT y C-LDL altos o moderadamente altos fue similar (p> 0,05) que los de aquellos alumnos de centros educativos privados. Un panorama similar se determin&oacute; en las adolescentes estudiantes de ambos tipos de colegios (<A HREF="#CUADRO3">Cuadro 3</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Discusi&oacute;n</FONT></FONT></B>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Las enfermedades carviovasculares, particularmente la cardiopat&iacute;a coronaria y la cerebrovascular son potencialmente prevenibles (<A HREF="#3">3</A>). Durante varios a&ntilde;os se han investigado los efectos de la modificaci&oacute;n de los factores de riesgo coronario; en la actualidad existe clara evidencia que al disminuir los niveles de CT, especialmente C-LDL, es posible retarsar y prevenir la aparici&oacute;n de cardiopat&iacute;a coronaria (<A HREF="#10">10</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Actualmente en pa&iacute;ses industrializados como los Estados Unidos y Finlandia, donde los niveles s&eacute;ricos de colesterol en j&oacute;venes son significativamente elevados, las tasas de mortalidad por enfermedad cardiovascular alcanzan noveles tan altos como 170 y 264 por 100.000 habitantes respectivamente (<A HREF="#2">2</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El elevado porcentaje de adolescentes con niveles altos o moderadamente altos de CT y C-LDL, evidenciados en este estudio, sugiere el posible impacto que este hallazgo pueda tener sobre la tasa de mortalidad por enfermedad cardio-vascular de los costarricenses adultos de las primeras d&eacute;cadas del siglo XXI. Este panorama se agudiza al considerar que los adolescentes incluidos en este estudio no presentaban sobrepeso u obesidad.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La situaci&oacute;n es m&aacute;s cr&iacute;tica en el caso de los adolescentes masculinos, pues un alto porcentaje (62%) de estos presentan tambi&eacute;n, niveles bajos o moderadamente bajos de C-HDL, la lipoprote&iacute;na responsable del transporte inverso del colesterol, un proceso asociado a la prevenci&oacute;n de la enfermedad coronaria (<A HREF="#14">14</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Para algunos investigadores, las determinaciones de C-HDL son m&aacute;s utiles para la predicci&oacute;n del riesgo de presentar una enfermedad de las arterias coronarias que el conocimiento de los valores totales de colesterol plasm&aacute;tico. No obstante resulta m&aacute;s conveniente a&uacute;n y tiene mayor valor predictivo el Indice de Castelli (C-T / C-HDL) (<A HREF="#15">15</A>), pues este cociente presenta un elevado coeficiente de correlaci&oacute;n con el colesterol LDL (<A HREF="#16">16</A>) por lo que resulta un excelente indicador del riesgo de sufrir enfermedad cardiovascular (<A HREF="#15">15</A>,<A HREF="#16">16</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Ante esta situaci&oacute;n, es alarmante observar el elevado porcentaje de adolescentes (56%) con valores mayores de 4,5 para el Indice de Castelli, pues estos presentan un mayor riesgo de sufrir coronariopat&iacute;as que aquellos j&oacute;venes con Indices de Castelli inferiores a 4,5 (<A HREF="#15">15</A>). Este hallazgo hace meditar sobre el posible impacto del mismo sobre una probable reducci&oacute;n, de la edad en la cual los costarricenses podr&iacute;an sufrir un infarto.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Epidemiol&oacute;gicamente se ha observado que los hombres adultos poseen un riesgo dos veces mayor que las mujeres adultas pre-menop&aacute;usicas de sufrir enfermedad cardiovascular (<A HREF="#11">11</A>), debido a los mayores niveles de C-HDL que estas presentan. Este comportamiento de los niveles de HDL, se refleja claramente en el estudio, lo cual se explica al considerar que los principales cambios en el perfil de l&iacute;pidos entre hombres y mujeres se originan en la pubertad. En esta etapa, los niveles de colesterol disminuyen; en los hombres se debe a la reducci&oacute;n del C-HDL, mientras en las p&uacute;beres los niveles de estas lipoprote&iacute;nas permanecen constantes como reflejo del incremento de estr&oacute;genos y adiposidad corporal que ocurre durante la madurez (<A HREF="#2">2</A>,<A HREF="#17">17</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los valores de l&iacute;pidos y lipoprote&iacute;nas plasm&aacute;ticas son el resultado de la interacci&oacute;n de complejos procesos metab&oacute;licos que se encuentran bajo control en un n&uacute;mero de influencias gen&eacute;ticas y ambientales. Aunque en este estudio no se contemplaron estas variables, algunos datos disponibles pueden contribuir a explicar el perfil lip&iacute;dico evidenciado.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Varios factores diet&eacute;ticos se han asociado al incremento de los niveles s&eacute;ricos de CT y C-LDL; sin embargo, el principal determinante nutricional que muestra un importante efecto sobre los niveles de estos l&iacute;pidos es la proporci&oacute;n de grasa saturada en la dieta (<A HREF="#2">2</A>,<A HREF="#3">3</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En Costa Rica, la dieta aporta alrededor del 13% del valor energ&eacute;tico total, en forma de grasa saturada, patr&oacute;n que se mantiene en la dieta de los adolescentes (<A HREF="#18">18</A>). Esta situaci&oacute;n podr&iacute;a explicar, al menos en parte, los elevados niveles de CT y C-LDL en los adolescentes estudiados, pues en pa&iacute;ses como Holanda, Filandia y Estados Unidos, donde el consumo de grasa saturada var&iacute;a entre 13,5 y 17,7% del consumo energ&eacute;tico diario, los niveles de colesterol total en ni&ntilde;os son frecuentemente mayores a 170 mg/dl (<A HREF="#2">2</A>). Un panorama diferente se observa en Italia, Filipinas y Ghana, donde la grasa aporta cerca del 10% o menos del consumo energ&eacute;tico diario, pues en estos pa&iacute;ses los niveles de colesterol en ni&ntilde;os de 7-9 a&ntilde;os son generalmente menores o iguales a 170 mg/dl (<A HREF="#2">2</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Algunos autores al analizar los factores de riesgo cardiovascular, consideran pertinente estudiar el nivel socio-econ&oacute;mico, pues este define muchas veces patrones alimentarios y estilos de vida que influyen en el perfil de l&iacute;pidos (<A HREF="#19">19</A>). En este estudio no se encontraron diferencias significativas entre el perfil l&iacute;pidico de estudiantes de colegios privados y de aquellos adolescentes que acuden a centros educativos p&uacute;blicos, a pesar de que se han observado diferencias, al menos en t&eacute;rminos de consumo de alimentos, entre estratos sociecon&oacute;micos. La segunda encuesta nacional de consumo aparente de alimentos muestra que a medida que aumenta el decil de ingreso econ&oacute;mico, disminuye el consumo de manteca de palma aficana (fuente de &aacute;cido palm&iacute;tico) y aumenta la ingesta de aceite vegetal (<A HREF="#20">20</A>), un alimento fuente de &aacute;cidos grasos mono y poliinsaturdos. Estos &aacute;cidos grasos tienen la propiedad de disminuir los niveles de C-LDL y ejercer un afunci&oacute;n antitrobog&eacute;nica cuando sustituyen la grasa saturada de la dieta (<A HREF="#21">21</A>,<A HREF="#22">22</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Este patr&oacute;n de consumo le brinda una connotaci&oacute;n econ&oacute;mica a dicha pr&aacute;c-tica alimentaria, sin embargo el porcentaje de grasa saturada en la dieta es similar en todos los deciles econ&oacute;micos (12-13% del total de grasa) (<A HREF="#20">20</A>). Ante esta si-tuaci&oacute;n y al no observarse diferencias en los niveles de l&iacute;pidos entre los adolescentes, podr&iacute;a asumirse que el porcentaje de grasa saturada que estos consumen es similar, aunque las fuentes de esta difieran.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Nuestros resultados sugieren la urgencia de una pronta atenci&oacute;n de las autoridades de salud sobre la poblaci&oacute;n adolescente, a fin de desarrollar programas preventivos que orienten a este grupo et&aacute;reo a modificar aquellos factores que incrementan los l&iacute;pidos sangu&iacute;neos y por ende tender a disminuir la tasa de mortalidad por enfermedad cardiovascular en el siglo XXI. Adem&aacute;s es necesario, recordar al personal m&eacute;dico la necesidad de estudiar el perfil de l&iacute;pidos en los adolescentes, principalmente cuando estos poseen familiares de primer grado de consanguinidad que han sufrido alguna enfermedad cardiovascular tempranamente.</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1><SUP>Abstract</SUP></FONT></FONT></B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>The lipidic profile of 204 non-obese healthy seventeen year-old adolescents, from four private and public high schools in the urban area of San Jos&eacute;, Costa Rica was studied. Women presented significantly higher levels of total cholesterol (TC), LDL cholesterol (LDL-C) and HDL cholesterol (HDL-C) (p&lt; 0,05) than men. There werw no statistical differences in TC, LDL-C, HDL-C between studens from private high schools and students from public high schools. 52% of men and 75% of women had total cholesterol levels considered elevated or moderately elevated. Smilary, 47% of male adolescents and 59% of female adolescents presented and LDL-C levels considered elevated or moderately elevated.
