Para el estudio se evalúan dos grupos de 14 mujeres cada uno, el primerointegrado por pacientes que habían sufrido un infarto del miocardio, con edad promedio de 55 años ± 11 y el segundo compuesto por mujeres que no tenían patología coronaria, con edad promedio de 49 años ± 9. La mayoría trabajaba en labores del hogar y el número de menopaúsicas fue semejante en ambos grupos; ninguna tomaba estrógenos sustitutivos. Los resultados mostraron una mayor prevalencia de hipertensión arterial, diabetres mellitus, antecedentes de infarto familiar y obesidad en el grupo que había tenido infarto. El consumo de tabaco no fue significativo en ambos grupos. Los triglicéridos se reportaron elevados significativamente (p< 0,005) en el grupo de mujeres con infarto con respecto al de mujeres sanas; también se observaron valores mayores en la relación colesterol total/colesterol HDL en el grupo con infarto. Los niveles de colesterol LDL, colesterol HDL y de lipoproteína (a) presentaron diferencias no significativas entre ambos grupos.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The modifable cardiovascular risk factors were analized: age and family coronary disease history; and not modifable: smoking, diabetes mellitus, hypertension, obesity and lipid profiles, including lipoprotein (a). Two groups of women were studied with 14 members each. The first group was composed of women were studied with 14 members each. The first group was composed of patients with a mean age of 55 years ± 11 that had coronary disease; the second group was composed of non coronary disease women, mean age 49 years old ± 9. Most of them were house wives. Postmenopausal members were almost the same in both groups, without estrogen-replacement therapy. The first group shows hypertension, and diabetes mellitus. It also shows hypertrigliceridaemia (p< 0,05) and the relationship between total colesterol/HDL colesterol was higher than the second group. There was no significance between, LDL, HDL and lipoprotein (a) levels.]]></p></abstract>

En la b&uacute;squeda de los posibles factores etiol&oacute;gicos de esta patolog&iacute;a, se debe considerar el actual estilo de vida, donde los componentes culturales y sociales tienen una decisiva relaci&oacute;n con la salud y la enfermedad e incluyen en los factores de riesgo cardiovascular modificables, los que aunados a una predisposici&oacute;n familiar, resultan en el proceso isqu&eacute;mico del miocardio.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La prevalencia de infarto en las mujeres no se aleja de la de los varones, sobre todo en la edad menop&aacute;usica, donde se presenta una disminuci&oacute;n de la protecci&oacute;n estrog&eacute;nica (<A HREF="#3">3</A>). En las que han presentado un evento coronario, las cifras de mortalidad alcanzan su m&aacute;ximo en e primer a&ntilde;o de post infarto, posteriormente el riesgo de morir es de 7% por a&ntilde;o en dicho sexo, mientras que en los hombres es de 5% (<A HREF="#4">4</A>). Es as&iacute; que se hace indispensable conocer c&oacute;mo se comportan los diferentes factores de riesgo cardiovascular en el sexo femenino, objetivo que se propuso en el presente estudio.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Materiales y m&eacute;todos</FONT></FONT></B>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Se estudiaron dos grupos de pacientes, uno de 14 mujeres que hab&iacute;an sufrido un infarto recientemente (en los dos &uacute;ltimos meses) y el otro de 14 mujeres sanas, esposas de pacientes con infarto; en ambos grupos estaban iniciando el programa de rehabilitaci&oacute;n carviovascular, el grupo sano en realidad de acompa&ntilde;antes de sus c&oacute;nyuges.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      ]]></body>
<body><![CDATA[<P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>A todas las pacientes se les tom&oacute; datos de edad, ocupaci&oacute;n, presencia o no de menopausia, ingesta de medicamentos que alteraran los l&iacute;pidos, antecedentes familiares, tabaquismo, y antecedentes patol&oacute;gicos personales; as&iacute; mismo se les tom&oacute; presi&oacute;n arterial, talla, peso y muestra de sangre en ayunas de 14 horas para el an&aacute;lisis de colesterol total, triglic&eacute;ridos, lipoprote&iacute;nas de alta densidad (HDL) y glicemia por medio de m&eacute;todos enzim&aacute;ticos colorim&eacute;tricos estandarizados. Tambien se evalu&oacute; el nivel de lipoprote&iacute;na (a) a trav&eacute;s de un m&eacute;todo inmunoenzim&aacute;tico. Las lipoprote&iacute;nas de baja densidad (LDL) se analizaron por la f&oacute;rmula de Friedwald. La evaluaci&oacute;n de la alimentaci&oacute;n se llev&oacute; a cabo con registro de tres d&iacute;as y la frecuencia de consumo de alimentos.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     <BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Resultados</FONT></FONT></B>     <CENTER><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B></CENTER> <FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La poblaci&oacute;n del estudio estuvo compuesta por 28 mujeres, 14 que hab&iacute;an tenido un infarto, cuya edad promedio es de 55 a&ntilde;os &plusmn; 11 y 14 mujeres sanas, con edad promedio de 49 a&ntilde;os &plusmn; 9. En el primer grupo hubo 10 (71%) en etapa menop&aacute;usica y en el segundo grupo 9 (64%); ninguna tomaba estr&oacute;genos sustitutivos. En ambos grupos a mayor&iacute;a de mujeres se desempe&ntilde;aban como amas de casa, el 64% del grupo con infarto y el 79% del grupo de las sanas.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El tabaquismo no fue significativo en ambos grupos, s&oacute;lo 10% consum&iacute;an cigarrillos (2 post-infarto y 1 sana).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En las pacientes con infarto hubo mayor prevalencia de antecedentes familiares de dicha enfermedad 86% contra 36%; as&iacute; como antecedentes personales de hipertensi&oacute;n, 57% versus 21%. Hab&iacute;a 14% de diab&eacute;ticas en el grupo con infarto y ning&uacute;n caso en el grupo de sanas.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En las 28 mujeres se observ&oacute; un promedio de IMC (&iacute;ndice de masa corporal) de 26. Entre las pacientes con infarto, 57% present&oacute; alg&uacute;n grado de obesidad (IMC > 27) contra 36% en el grupo de las sanas (<A HREF="#CUADRO1">cuadro 1</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Los triglic&eacute;ridos s&eacute;ricos estuvieron elevados (m&aacute;s de 200 mg/ dl) en forma significativa (p &lt; 0,05) en 93% de las pacientes que hab&iacute;an presentado infarto con respecto a 57% del otro grupo (<A HREF="#CUADRO2">cuadro 2</A>); la relaci&oacute;n colesterol total / colesterol HDL tuvo niveles altos (m&aacute;s de 4,5) en 64% de las pacientes con infarto y s&oacute;lo en 29% de las pacientes sanas (<A HREF="#CUADRO3">cuadro 3</A>). La diferencia en los valores de LDL no fue significativa en ambos grupos; sin embargo, 42% de pacientes con infarto y 57% sanas presentaron niveles por encima de 130 mg / dl. Las HDL (<A HREF="#CUADRO4">cuadro 4</A>), as&iacute; como los niveles sangu&iacute;neos de lipoprote&iacute;na (a) tampoco presentaron diferencias significativas. Esta &uacute;ltima al ser analizada en menopa&uacute;sicas y no menopa&uacute;sicas, sin tener en cuenta la presencia de infarto, se observ&oacute; que 89% de las primeras ten&iacute;an valores elevados (m&aacute;s de 20 mg / dl) contra 55% de las no menopa&uacute;sicas.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El contenido de grasas saturadas y colesterol en la dieta fue adecuado en ambos grupos; la modificaci&oacute;n de los h&aacute;bitos alimentarios lo indicaron inmediatamente despu&eacute;s del infarto por consejo m&eacute;dico; lo que fue adquirido tambi&eacute;n por el grupo de sanas, al contribuir con la rehabilitaci&oacute;n de sus c&oacute;nyuges. El patr&oacute;n de alimentaci&oacute;n mostr&oacute; una adecuada frecuencia de consumo de frutas y vegetales y un bajo consumo de carnes y grasas.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT>     ]]></body>
<body><![CDATA[<BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Discusi&oacute;n</FONT></FONT></B>     <CENTER><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></CENTER> <FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La enfermedad coronaria es la causa m&aacute;s com&uacute;n de mortalidad en hombres y mujeres, y aunque tiene mayor prevalencia en los primeros, &eacute;sta se iguala en la etapa menopa&uacute;sica; a&uacute;n as&iacute;, la mayor&iacute;a de los estudios de prevenci&oacute;n primaria realizados en los decenios de 1970 y 1980 han sido en varones de edad media de la vida, y los resultados se han aplicado muchas veces a los dos sexos (<A HREF="#3">3</A>). Por otro lado en Estados Unidos la mitad de las defunciones por infarto agudo del miocardio en cualquier a&ntilde;o, en particular, se produce en mujeres (<A HREF="#6">6</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En el presente estudio, los antecedentes familiares de infarto del moicardio, as&iacute; como los personales de diabetes, hipertensi&oacute;n y obesidad, fueron factores de riesgo cardiovascular relevantes en las mujeres que hab&iacute;an presentado un infarto. El consumo de tabaco fue bajo en los grupos y el tama&ntilde;o de la muestra impide valorar adecuadamente &eacute;ste factor de riesgo.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Es indudable la relaci&oacute;n del peligro de un evento coronario, cuando existe el antecedente familiar del mismo, sobre todo si se ha presentado en edad temprana (<A HREF="#7">7</A>), tambi&eacute;n existen datos de que la protecci&oacute;n que las mujeres tienen contra la aterosclerosis se pierde casi por completo cuando ellas presentan diabetes (<A HREF="#8">8</A>); en cuanto a la hipertensi&oacute;n, por si sola, al parecer no induce a aterog&eacute;nesis, pero en presencia de un nivel cr&iacute;tico del colesterol LDL, acelera extraordinariamente la aterosclerosis (<A HREF="#9">9</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La obesidad medida por el IMC (&iacute;ndice de masa corporal), se ha relacionado con peligro de enfermedad cardiovascular, si est&aacute; por encima de 27 (<A HREF="#10">10</A>); sin embargo la distribuci&oacute;n del peso corporal, es tal vez m&aacute;s importante en cuanto a predecir el riesgo; as&iacute;, las mujeres cuyo incremento ponderal se manifiesta en la cintura est&aacute;n en mayor peligro de infarto que aquellas en el incremento se observa a nivel de caderas (<A HREF="#11">11</A>). En el an&aacute;lisis del perfil lip&iacute;dico de los grupos estudiados, la elevaci&oacute;n de los triglic&eacute;ridos fue estad&iacute;sticamente significativa en el grupo que hab&iacute;a presentado un infarto; con respecto al otro grupo, la relaci&oacute;n colesterol total / colesterol HDL present&oacute; tambi&eacute;n mayores valores en el grupo con infarto. Se observ&oacute; diferencia estad&iacute;sticamente no significativa en cuanto a los niveles de HDL, LDL, y lipoprote&iacute;na (a).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Existen publicaciones que expresan que el colesterol total y el colesterol LDl guardan relaci&oacute;n con la morbilidad y mortalidad por enfermedad cardiovascular en las mujeres (<A HREF="#12">12</A>); sin embargo, es poco probable que el colesterol total sea representativo, porque en las mujeres una proporci&oacute;n mayor de la cantidad total lo constituye el colesterol HDL. En las premenopa&uacute;sicas y en las menopa&uacute;sicas con sustituci&oacute;n estrog&eacute;nica, las LDL quiz&aacute;s no sean un factor de riesgo tan potente como en los varones, por los efectos antioxidantes del estr&oacute;geno (<A HREF="#13">13</A>). En cambio los triglic&eacute;ridos, que guardan s&oacute;lo relaci&oacute;n d&eacute;bil con el mayor riesgo de coronariopat&iacute;a en varones, constituyen un indicador m&aacute;s potente de peligro en mujeres (<A HREF="#14">14</A>). Se ha visto que los niveles bajos de HDL constituyen un elemento m&aacute;s potente de predicci&oacute;n en las mujeres que en el sexo masculino (<A HREF="#11">11</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>La lipoprote&iacute;na (a), una mol&eacute;cula que incluye otra de LDL unida por un enlace disulfuro a un p&eacute;ptido que tiene una similitud de 80% con el plamin&oacute;geno y que es captada en la pared arterial ha sido relacionada con enfermedad cardiovascular en hombres y mujeres cuando los niveles sangu&iacute;neos de la misma est&aacute;n por encima de 20 mg/dl (16). En el presente estudio no hubo significativa diferencia entre ambos grupos. La elevaci&oacute;n que se reporta en la lipoprote&iacute;na (a) en las mujeres menop&aacute;usicas probablemente se deba al d&eacute;ficit estrog&eacute;nico, pues se ha visto que los estr&oacute;genos la disminuyen. Actualmente hay una tendencia creciente hacia la sustituci&oacute;n estrog&eacute;nica en la menopausia, y una raz&oacute;n de peso es disminuir el riesgo de coronariopat&iacute;a (17) por otro lado las pacientes que ya han tenido un infarto est&aacute;n en mayor riesgo de un segundo evento coronario; sin embargo ninguna del presente estudio recib&iacute;a sustituci&oacute;n, probablemente &eacute;sto podr&iacute;a reflejar desconocimiento o temor a las reacciones adversas de &eacute;stos medicamentos.</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>El consumo de grasas saturadas y colesterol que se ha relacionado con el riesgo de infarto, no se evalu&oacute; en las pacientes del estudio debido a que ya hab&iacute;an variado sus h&aacute;bitos alimentarios y el mismo se consider&oacute; dentro de lo que actualmente se recomienda (<A HREF="#6">6</A>).</FONT></FONT><FONT FACE="Arial,Helvetica"><FONT SIZE=-1></FONT></FONT>      <P><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>En conclusi&oacute;n, hay factores de riesgo que sirven para predecir el peligro de problemas coronarios en mujeres y varones, pero otros se comportan de manera diferente en el sexo femenino como los niveles de LDL que tal vez no sean tan predictivos del riesgo; en cambio, los HDL pueden ser m&aacute;s predecibles. <body><![CDATA[<BR><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>Abstract</FONT></FONT></B>     <CENTER><B><FONT FACE="Arial,Helvetica"><FONT SIZE=-1>&nbsp;</FONT></FONT></B></CENTER> <FONT FACE="Arial,Helvetica"><FONT SIZE=-1>The modifable cardiovascular risk factors were analized: age and family coronary disease history; and not modifable: smoking, diabetes mellitus, hypertension, obesity and lipid profiles, including lipoprotein (a). Two groups of women were studied with 14 members each. The first group was composed of women were studied with 14 members each. The first group was composed of patients with a mean age of 55 years &plusmn; 11 that had coronary disease; the second group was composed of non coronary disease women, mean age 49 years old &plusmn; 9. Most of them were house wives.
