Based on a qualitative design, 15 nurses participated in the apparent validation of the instrument. No contributions were recorded regarding the writing and logical sequence of the information. Three items were added. The foregoing, considering that the ideas expressed were in accordance with the principles of voluntary reporting, non-punishability, confidentiality, systemic orientation, promotion of analysis for learning and implementation of improvements.
