One cup of coffee and other alternative therapies in clinical medicine Coffee is a drink with a very rich history, beginning with a legend about its discovery in the highlands of Ethiopia.Beneficial physiologic effects of coffee are well documented, and unlike many substances, coffee poses no increased risk for mortality. On the contrary, a possible mortality benefit has been reported.Patients with coronary heart disease have little reason to restrict their intake of coffee as a significant arrhythmia is usually only associated with consumption of very high quantities.Evidence also suggests that coffee decreases the incidence of diabetes mellitus type 2.As our understanding of the mechanisms of atherosclerosis continues to progress, we have learned that coffee may have benefit in regeneration of damaged endothelium, including the damage caused by inflammation.In one unique situation-a human genotype with slow caffeine metabolism-coffee appears to somewhat augment the incidence of acute myocardial infarction.With these facts in mind, as with alternative medications, it is important for the physician and cardiovascular specialist to understand the various effects of coffee, which is used by many of our patients.
