A 36 year old female, who at age 14 underwent a pulmonary valvulotomy for congenital pulmonary stenosis, presented with chest pain and dyspnea with mild activity for the last 6 months. Coronary angiography revealed the left coronary artery originating from a common vessel with the right coronary artery, and severe stenosis at the origin of the left anterior descending artery. The patient was referred for surgical coronary revascularization.
