Resumen

Objective: To show the usefulness of the Barber and Johnson diagram to analyze comparatively the productivity and management of hospital bed use in centers of National Health System in Costa Rica. Methods: Hospital beds, inpatient registers and total patient days for 2011 come from Caja Costarricense de Seguro Social. Barber and Johnson Diagram were used to compare indicators of length of stay, occupancy rate, turnover interval and discharges per bed in a year. Efficient centers were considered when the occupancy rate is less to 85% and the discharges per bed in a year are in a range between average plus or minus one standard deviation. Results: In 2011, the public hospitals have 5 613 available beds that hosted 323 thousand patients, with a consumption of 1,73 million days of stay. Eight hospitals are located in the efficiency area. This group is composed of one national hospital, six regionals and one low-complexity hospital. Conclusion: The Barber and Johnson diagram is a tool to assess graphically the relationship of four traditional indicators of management and productivity of the bed, in order to identify areas for improvement in the relations of medical and administrative work in the use of the hospital bed.
