The questionnaire is extensively used on the health care sector to collect information, and there has been discussion on the magnitude, extension and sources of the bias that could compromise the validity of the information; however, awareness is limited on the magnitude and diversity of this topic. This article present the most relevant health sector questionnaire bias based upon a previous work by Choi and Pak. Objectives: These article intents to discuss mechanisms and dynamics of potential bias in health sector questionnaires. Methods: There was an extensive literature review, to identify examples of bias from questionnaires used in different projects available in print format or on electronic format at web sites of public access. Results: 48 types of questionnaire biases were identified and discussed. We present alternatives to minimize or avoid bias, and a summary table is presented that provides a useful checklist for determining and preventing or minimizing possible sources of questionnaire bias. Discussion: The questionnaire is a common tool for health care activities, however, little formal capacity is provided on the topic at most specialties within the health sector. This article allows the reduction on bias that could compromise data validity, presenting the bias possibilities in a useful and accessible way for the non-expert in the topic
