Introduction: Trichophyton rubrum is worldwide the most frequent isolated dermathophyte, affecting skin, fingernails and toenails. Fluconazole is commonly used to treat onychomycosis in our population, thus, the objective of the present study was to study the susceptibility of T. rubrum costarrican isolates to this antifungal. Methods: The in vitro susceptibility to fluconazole of 80 isolates of T. rubrum taken from skin and its annexes was investigated using the microdilution method M-38, as described by the National Committee for Clinical Laboratory Standards. The final fluconazole dilutions were from 0.25 to 128 μg/ml. Results: The majority of T. rubrum isolates were obtained from toenails (68.75%). Among the isolates studied, 86.25% showed a minimal inhibition concentration between 0.25-8 μg/ml, 8.75% between 16-32 μg/ml and 5% > 64 μg/ml. Two out of the four resistant isolates were obtained from toenails, and two from the sole. Conclusion: Since the majority of T. rubrum isolates were susceptible to fluconazole, we recommend performing susceptibility tests only in cases of therapeutic failure, especially in patients with onychomycosis for whom treatment may last for several months.
