Early Arthritis

Hasta la década de los años ochenta se consideraba a la artritis reumatoide (AR) como una enfermedad poco frecuente, de gravedad leve a moderada, que tenía una evolución lentamente, progresiva hacia el daño articular y la incapacidad. El aborde terapéutico convencional hasta ese momento, era el tratamiento clásico de la pirámide.

Until the early the eighties was considered rheumatoid arthritis to (RA) as a rare disease of mild to moderate severity, which had a slowly evolution towards joint damage and disability. The conventional therapeutic option until then, was the classic treatment of the pyramid.

Si la meta terapéutica de la AR es lograr y mantener el menor grado de actividad o incluso de remisión, el manejo de la AR debe de incluir una evaluación cuantitativa clara de la inflamación de la artritis reumatoide en forma sistemática y regular. Debido a la alta variabilidad de la actividad de la AR, como también de la diversidad de sus otras manifestaciones, no es posible que una simple medición pueda realmente capturar la actividad global de la enfermedad.

If the goal of RA therapy is to achieve and maintain the lowest level of activity or even remission, the management of RA should include a clear quantitative evaluation of inflammation of rheumatoid arthritis in a systematic and regular. Due to the high variability of the activity of RA, as well as the diversity of its other manifestations, it is possible that a simple measurement can really capture the overall activity of the disease.

Disease Modifying Antirheumatic Drugs (DMARD)

El nuevo paradigma terapéutico de la artritis reumatoide establece que todo paciente con actividad de la enfermedad debe recibir alguna droga modificadora de la artritis reumatoide. Estas drogas reciben este nombre ya que han demostrado en estudios clínicos controlados modificar el curso natural de la enfermedad.

The new paradigm for the treatments of rheumatoid arthritis stablish that all patients suffering disease this should receive some drugs which modifys rheumatoid arthritis. These drugs are so named because according to the controlled clinical studies they have shown the ability of modifying the natural course of the diseases.

Biological Agents in the Treatment of Rheumatoid Arthritis

El mayor conocimiento inmuno-patológico de la artritis reumatoide permitió mediante una sofisticada tecnología molecular elaborar antagonistas contra blancos específicos en diversos pasos de la inmuno patogenia de la enfermedad. Existe una amplia evidencia sobre la eficacia, tolerabilidad y seguridad de estos agentes en diferentes escenarios clínicos de la artritis reumatoide. La introducción de estos agentes biológicos en el arsenal terapéutico de la artritis reumatoide ha marcado un hito en su evolución y pronóstico artritis reumatoide.

A better understanding of the immuno-pathological rheumatoid arthritis enabled by sophisticated molecular technology to develop antagonists against specific targets in various steps of the immune pathogenesis of the disease. There is ample evidence on the efficacy, tolerability and safety of these agents in different clinical scenarios of rheumatoid arthritis. The introduction of biological agents in the armamentarium of rheumatoid arthritis has marked a milestone in its evolution and prognosis rheumatoid arthritis.
